Key Responsibilities:
- Manage a caseload of young people, ensuring high-quality care and support.
- Develop care pathways and effectively signpost young people to appropriate mental health services.
- Provide comprehensive assessment and intervention for children and young people experiencing moderate to severe mental illness.
- Support the duty clinician and respond to urgent cases as required.
- Offer consultation, advice, training, and support to partnership agencies and colleagues within the multi-disciplinary team.

Main duties of the job
- You will be responsible for developing and delivering a high standard of assessment and treatment for patients referred to the service. This includes managing an agreed clinical caseload, ensuring continuity of care through effective care coordination as needed.
- In collaboration with patients, carers, and clinical teams, the clinician will assess and provide evidence-based treatment programmes and discharge plans, utilising the Care Programme Approach (CPA) to ensure a comprehensive approach to patient care. You will also plan and provide appropriate therapy services, using evidence-based practice, while adhering to defined resources.
- This role involves developing and maintaining relevant, evidence-based therapeutic activities tailored to the specific needs of the patient. Regular review and evaluation of treatment programs will be carried out in conjunction with patients, carers, and clinical team members to ensure the effectiveness of the care provided.
- You will actively participate in clinical meetings, case conferences, and CPA meetings, facilitating the smooth transition of patients through various stages of their care, including care planning and therapy programs. Additionally, you will be responsible for compiling written reports as required and ensuring that patient documentation is accurate and up to date.
