Job summary

Do you want to work in an integrated setting with colleagues from primary care, mental health services, including the voluntary and community sector, to improve access to support for people with mental health issues?

This role is eligible for an incentive scheme of £3,500 paid at intervals over an 18-month period. All payments are pro-rata, non-pensionable and subject to a clawback clause. Please note that only employees external to Oxford Health NHS are eligible for this scheme.

As part of our services across Oxfordshire, we are implementing Primary Care Mental Health hubs to support GP practices with Mental Health assessment and support for patients in their communities.

We have created new and exciting posts within Keystone Mental Health and Wellbeing Hubs. This role will be based within the Keystone Mental Health and Wellbeing Hub in Banbury.

Main duties of the job

The responsibilities of the role include, but are not limited to:

  • Supporting the Clinical Lead in the development and implementation of the service and your role within it.
  • Supervision and leadership for appropriate staff identifiedScreen, triage, assess and coordinate referrals
  • Support patients ready to transition to primary care and develop care plans and prepare people for step down.
  • Map community resources and assets that are available for those with SMI within the specific PCN geography.
  • To support collaborative working between the full range of physical health and mental health services

Knowledge
