Are you a newly qualified Social Worker registered with Social Work England?

Join our community mental health team in High Wycombe, Buckinghamshire, and work with adults in a supportive and experienced social care environment.

During your assessed and supported year in employment (ASYE), you'll benefit from a reduced caseload, dedicated supervision, and the Buckinghamshire Council ASYE programme. Your role will involve assessing needs, reviewing care packages under the Care Act and s117 MHA, and visiting service users in their homes and placements. You'll work closely with multi-disciplinary teams while receiving direct support from the dedicated social care structure. This position offers the opportunity to develop your skills and knowledge within a structured and supportive setting.

Main duties of the job

  • Undertake Care Act assessments for adults and their carers who are ordinarily resident in Buckinghamshire and appear to be in need of care and support.
  • Using a strengths-based approach you will help adults and their families make best use of community resources to support them to live as independently as possible.
  • Arrange for commissioned services to be put in place where necessary, in line with eligibility and Council policy.
  • Review existing packages of care in line with the Care Act and s117 Mental Health Act 1983.
  • You will develop a good understanding of the Council's charging and top-up and choice policies and give information and advice to adults and their families when assessing and arranging care and support.
  • Work jointly with Social Worker and health colleagues to deliver a supportive and empowering service to adults and their families.
