Oxford Health NHS Trust

The closing date is 04 November 2024

Job summary

Join our team as a Mental Health Pharmacist in this exciting developmental post at Oxford Health NHS Foundation Trust. We are looking for a dedicated and enthusiastic pharmacist to deliver innovative clinical pharmacy services to in-patients, out-patients, and day-patients. You'll work closely with a multi-disciplinary team to optimise drug therapy, ensuring the best outcomes for patients across Oxfordshire.

The role covers specialities including inpatient adult, older adult, children and adolescent units, as well as working with the Directorate Lead Pharmacist to develop our community-based mental health team, which is in its infancy. This is a patient-facing role where you'll be working with the healthcare team on clinical sites. There are times where remote working can be facilitated and we can offer a variety of flexible working solutions, including full, part-time, and annualised hour contracts.

This is an excellent developmental opportunity for experienced pharmacists to elevate their career and gain experience within hospital pharmacy and mental health care.

If you are registered with the GPC and have experience in, or have a passion for hospital pharmacy, then we'd love to hear from you!

The ability to travel freely between sites across the Trust is essential for this role. Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required.

Main duties of the job

In this role you'll be:

  • Working alongside clinical pharmacy colleagues across the Mental Health Directorate
  • Responsible for the development of strong clinical relationships with the patients, carers and healthcare professionals in your in-patient and community settings.
  • Directly supporting patients, prescribers, and other healthcare professionals via attendance at multi-disciplinary team (MDT) meetings and one-to-one patient consultations, carry out medication reviews, engage in discharge-planning with the MDT and ensure the correct application of national and local guidance with regard to therapies involving medicines.
  • Supporting and working alongside our centralised hub pharmacist where necessary (based at the Clinical Pharmacy Support Unit).
  • Supporting the ongoing development of the community based pharmacy services and departmental clinical effectiveness initiatives.
  • A member of the on-call pharmacist out of hours team as per rota where applicable.
  • Encouraged and supported to professionally develop in line with the departmental and your own personal needs as appropriate.

Please refer to the job description attached for a comprehensive list of duties
