Community nursing is a challenging and rewarding career, providing care for patients within their own homes and the community setting.

Main duties of the job

District nurses make a difference everyday to the lives of the people they visit at home and in residential care homes. They provide increasingly complex care for patients and support for family members.

You will visit patients in their own homes and provide a comprehensive range of clinical nursing activities. A key aspect of the role will involve working with the wider multidisciplinary team to help plan and deliver complex care to enable patients to remain in their home environment for as long as possible.

Working as part of an integrated locality team, you will work in partnership to reduce hospital admission and facilitate discharge by providing clinical holistic assessment and treatment.

You will promote optimal levels of independence and wellbeing for patients whilst facilitating teaching and support for formal and informal carers.

About us

You will have access to the Oxford Health Flyer Programme designed to support and develop newly qualified nurses and allied health professionals (AHP)s.

It has been recognised as an exemplary programme for two consecutive years with Health Education England (HEE) awarding Oxford Health the Capital Nurse preceptorship quality mark 2020-2021. The trust is one of 24 to achieve the status and is the only organisation to do so outside of London.

The programme provides evidence-based support and guidance to all newly registered nurses and AHPs helping them make the transition from studentto professional, growing their confidence as well as refining skills, values and behaviours.

In the first-year, participantsreceive nine days' training and support on topics such as leading care, patient safety and action learning.'Flyers' have the option to accredit this year and gain 20 credits at Masters level via Oxford Brookes University.

The second year continues learning and professional development, with careers discussions to provide staff individualised career pathways.
