Job summary
An exciting opportunity has arisen for experienced mental health clinicians to work within in a newly created service improving the lives of service users requiring long term care and placement.
The discharge liaison service is beingestablishedin order toimprove thedischarge pathway forpatients requiring24 hourplacement, wheredischarge is delayed due to difficulties inidentifyinga care setting with thecapacity,capabilityand competencyto manage theirparticular complexmental health needs.
The clinicians in this service will work closely with inpatient settings and care homes, supporting patients for up to 6 weeks post discharge.
The post holders will work alongside the Care Home Support Service which aims to improve the quality of life of care home residents, reduce hospital admissions and facilitate early discharge from hospital.
The Service is county wide and the post holders may be asked to cover any localities within Oxfordshire and potentially with very complex cases outside of Oxfordshire.
Main duties of the job
The discharge liaison clinicians will focus on supporting discharge from acute mental health wards to a care home setting, in particular supporting the transition from hospital setting to long term care and working alongside care home staff to support and ensure successful and sustainable placements.
You will be working with adults who have a dementia and/or a functional mental illness who meet the criteria for permanent placement in a care home. Your involvement will start whilst they are in an inpatient setting
The team will also support complex discharges of patients under psychological medicine in the acute hospital system.
You will be working closely with care home staff in enabling them to understand the needs of the patient and how best to meet these within the care setting so as to enable the placement to be successful.
To provide specialist advice, support and consultancy to health professionals, patients, and relatives.
