Job summary

This is an exciting opportunity to be part of a small, specialist service that works under the For Me Provider Collaborative (Thames Valley and Wessex adult secure PC). The Forensic Intellectual and Neurodevelopmental Disabilities Service (FINDS), works alongside other services who are supporting individuals with a current forensic need and a diagnosis of either/or learning disabilities, Autism and/ or acquired brain injury by offering consultancy, training, and advice, as well as the gatekeeping into secure inpatient services within the For Me footprint.

Main duties of the job

The successful candidate will manage this small, but experienced, team of specialist health professionals. They will ensure that the team work within the principles of the Mental Health Act, the Mental Capacity Act and the NHSE secure service specifications to ensure that those referred are offered the most appropriate pathway outcomes.

They will support the team to deliver training and consultancy, liaise with various services across the footprint, including the case managers within secure hospitals, the ICB, community mental health and learning disability community teams, social care, prisons and other none secure hospitals.

In addition, you will be responsible for chairing meetings which include FINDS and other members of the provider collaborative and being part of service improvements that provide better outcomes for this cohort.

The role of FINDS is to enhance the skills and knowledge of providers to reduce the need for secure hospital, follow the person through their stay in secure offering support around reasonable adjustments if required and to offer teams supporting the person on discharge training and understanding around the persons needs for a 6 month period to reduce readmission to secure hospital.

This role will support the team in the assurance of specialised services including guidance around Transforming Care, by providing the link with Clinical Commissioning Groups (CCGs) and their commissioned services within the network to ensure there is a coordinated approach to care and discharge planning, which results in robust and effective care packages

You will have oversight of the overall patient pathway, supporting strong multi-agency involvement to ensure patients are receiving care in the least restrictive environment and barriers to discharge are expediently addressed

You will be expected to model a collaborative and influencing style of working and actively engage in clinical and professional supervision and to uphold organisational principles and policies on the promotion of equality.

You will also provide education and support for others involved in the care of patients within specialised settings, working to reduce stigma and bias for patients working across multiple providers within the NHS and Independent Sector as well as with services within the Thames Valley and Wessex region to manage relationships with CCG commissioned providers that provide the referral and aftercare components of the pathway.

A strong communicator you will be tasked with drafting reports summarising status on issues, appraising outcomes, and providing progress reports to the Senior Clinical Pathway Manager, and plan, develop and evaluate methods and processes for gathering, analysing, interpreting and presenting data and information.
