Job summary

We currently have an exciting opportunity to join the Personality Disorder Inpatient Transition Team (PDITT) at Oxford Health.

The PDITT is a pilot project funded until April 2025, and we are looking for a highly motivated and enthusiastic individual to fill the role of Mental Health and Wellbeing Practitioner on a fixed-term basis.

The successful applicant will work collaboratively with patients, their families and significant others, alongside the patient's Keyworker and all professionals involved in their care, to support a safe transition between the hospital and community for patients diagnosable with Personality Disorder.

Main duties of the job

  • Offer support to patients throughout their hospital admission. This support will involve face-to-face interaction with the patient, as well as their family and significant others.
  • Work in collaboration with all professionals involved in their care.
  • Support individuals in creating effective, personalised, crisis and safety plans.
  • Identify any non-clinical barriers to discharge and collaborate with the appropriate professionals to address these.
  • Attend ward rounds and discharge planning meetings, and actively participate in professional meetings and multi-disciplinary discussions.
  • Work alongside other team members to provide service-users with intensive follow-up for up to one month after they have left hospital.

Qualificaton

Essential

  • A graduate certificate (Level 6) or postgraduate certificate (Level 7) in specialised area (Social Work, Occupational Therapy or Nursing).
  • Registration with relevant professional body.

Experience

Essential

  • Experience of working with people with mental health needs, gained through a graduate certificate or postgraduate certificate training with significant supervised practice. Experience of working with people with Personality Disorder.
  • Experience of working with people with Personality Disorder.
  • Experience of working in a team.

Desirable

  • Experience of working in a Crisis Team.

  • Able to establish and maintain empathetic, supportive relationships with people in significant distress or who are cognitively impaired, their families and carers.
