Job summary

As Senior Clinical Pathway Manager in the CAMHS (Thames Valley) Tier 4 Provider Collaborative, you will work as part of a dynamic team in delivering an effective service to ensure safe care pathways are in place for our service users.

Working as part of the CAMHS Tier 4 team, responsible for the provision of care for specialised mental health, you will provide clinical support to case managers with designated caseloads to enable:

  • the ongoing therapeutic interventions of service users in specialised health care.
  • oversight of access assessments.
  • ensuring service users are placed in the least restrictive setting with the appropriate level of supportive interventions to enable their discharge into the community where this is a safe option or maintain their individual needs if they remain in a care setting.

You will be key to ensuring people have their physical health needs met, oversight of Care and Treatment Review processes, managing the use of out of natural clinical flow placements and driving transformation as well as value for money in planning, commissioning, and service delivery.

Main duties of the job

The Provider Collaborative represents a partnership approach to the redesign of the CAMHS Tier 4 care pathways across the designated scope of the network. You will need proven and sophisticated skills in managing relationships with clinicians and operational leads across different organisations, within and beyond the network, and advanced knowledge of the CAMHS commissioning landscape and care pathways.

You will have responsibility for providing clinical leadership, management, and supervision to a team of CAMHS Case Managers, ensuring that safe and effective care pathways are in place for all Provider Collaborative patients in CAMHS Tier 4 mental health care settings, within the NHS and Independent Sector that are consistent with the aims and strategy of the Provider Collaborative.

You will take a lead role in the clinical assurance of the quality of the care provided to the patients of the Provider Collaborative - working with Case Managers and in partnership with NHSE / ICBs as required.

You will also provide a link to PLACE / ICSs and other stakeholders and interfaces with the Provider Collaborative to ensure there is a co-ordinated approach to care pathway and discharge planning, supporting robust and effective care packages.
