Main duties of the job

The Clinical Nurse Lead for Physical Health Care in Community MH Teams will spearhead the development of physical health processes, provide leadership to MH community teams, and deliver high-quality health assessments and care plans for individuals with Mental Illness.

This role involves strategic planning, training, and collaboration with multidisciplinary teams to ensure the overall well-being of patients. Essential skills include clinical leadership, strategic thinking, and a commitment to continuous improvement.

The Clinical Nurse Lead for Physical Health Care in Community MH Teams will be a registered nurse with experience in community settings and possesses the ability to engage in research and clinical audit.

As the Clinical Nurse Lead for Physical Health Care in SMI A&OA Community MH Teams (Band 8a), your main responsibilities include:

  1. Leadership and Development:
    • Lead the development of processes and pathways for improving the physical health of SMI patients.
    • Provide clinical leadership and collaborate with multidisciplinary teams in adult and older MH community services.
    • Deliver specialized training on physical health and health promotion to clinicians and healthcare professionals.
  2. Strategic Initiatives:
    • Lead the strategic development and delivery of health promotion and physical health care within the service.
    • Develop and implement a health promotion strategy.
    • Contribute to advanced practice initiatives, education, and training.
  3. Clinical Oversight:
    • Provide professional and managerial supervision to physical health nursing roles.
    • Lead a team of physical health Health Care Assistants within each community team and Physical Health Outreach.
    • Undertake clinical work, training, and personal development to maintain and develop clinical and leadership skills.
    • Contribute to complex case discussions and investigations of serious incidents.
  4. Training and Education:
    • Develop, co-ordinate, and contribute to the delivery of physical health and well-being training programs.
    • Provide relevant training to Practice Nurses and other healthcare professionals within PCNs.
    • Contribute to research activities as required.
  5. Health Assessment and Care Plans:
    • Provide advice and contribute to the delivery of high-quality Physical Health Assessments for SMI patients.
    • Lead the development and delivery of nursing care plans advising on physical healthcare, including lifestyle, diet, and exercise.
  6. Collaboration and Networking:
    • Develop and maintain good links with other community services, Primary Care, and partner organizations.
    • Collaborate with other clinical nurse leads for physical health within the trust.
    • Ensure equitable access to NHS screening provisions for patients with SMI.
  7. Audit and Reporting:
    • Lead on clinical audit related to physical health.
    • Produce quarterly reports providing an overview of physical health care standards within the service.
  8. Compliance and Codes of Conduct:
    • Ensure compliance with Nursing and Midwifery Council Code of Professional Conduct.
    • Adhere to the Professional Code of Conduct and NHS Code of Conduct for Managers.
    • Uphold principles of equal opportunities, diversity, health and safety, and infection control.
  9. Personal and Professional Development:
    • Participate in annual performance reviews and develop personal development plans.
    • Engage in continued professional development and training activities.
