The City Adult Mental Health Team (AMHT) is excited to announce a further rollout of our newly established Senior Care-Co-ordinator roles!

If you're a Registered Mental Health Nurse, or an Occupational Therapist, looking for a role where you can really contribute to improving the outcomes for adults with mental health conditions, then this is a fantastic opportunity for you. These band 7 roles have been created to enable the right candidate to advance their career in a more clinically focused way but to still include operational management opportunities.

The City AMHT supports adults of working age (18-65) who experience a wide range of mental health conditions. We are committed to the person-centred recovery approach in all our clinical work and pride ourselves in offering a high-quality service reflected in our CQC rating of Good.

You will clinically lead a small team consisting of newly qualified Nurses/Occupational Therapists, Nurse Associates, Peer Support Workers and Support Workers to provide care and interventions to a complex caseload of patients. This will be as part of a wider team working alongside 2 - 3 primary care networks, supported by both psychiatric and psychological therapy clinicians.

Main duties of the job

You will act as an advanced autonomous practitioner, demonstrating a high level of practice/knowledge whilst participating in direct evidenced based patient care working as part of a multi- disciplinary team providing professional and clinical support to patients.

You will undertake autonomous health assessments; diagnosing, treating and discharging patients within own competence. The Senior Care Coordinator will provide expert care and treatment to patients within agreed protocols and clinical guidelines.

You will provide support within the team and act as role model and resource, encouraging and developing a high standard of clinical skill and improving patient outcome

You will function in a variety of role dimensions: health care provider, educator, researcher, clinical advocate and implementer of clinical governance. The Senior Care Coordinator will lead a small team of newly qualified nurses/ OTs, Nurse Associates, Peer Support Works, Support Workers to provide Evidence based care and interventions to a Complex Caseload Care Coordinated by the Senior Care Coordinator

You will be a senior clinician within the team and lead a small team of newly qualified Nurses/Occupational Therapists, Nurse Associates, Peer Support Workers and Support Workers to provide care and interventions to a complex caseload of patients. The role be predominantly clinical, but also include some operational leadership and supervision to your small team of junior staff.

Job responsibilities

The City and Northeast Oxon Adult Mental Health Team's main base is at the Warneford Hospital, a stones throw from the centre of Oxford and the dreamy spires. We provide care for the residents of Oxford, the local towns of Kidlington, Bicester and Woodstock, and the surrounding villages. The team is split into three Flexible Assertive Community Treatment (FACT) teams; Willow, Oak, and Aspen, which are aligned to local Primary Care Network patches.

We use the principles of a flexible assertive community treatment (FACT) model of healthcare. We have incorporated FACT into the daily functioning of the team; which enables us to be focused on the recovery of the patient through a multidisciplinary (MDT) approach; with expertise from doctors, social workers, nurses, occupational therapists, support workers and psychological therapies staff.

Your normal working week will be Monday to Friday 9:00-17:00, although some trust services operate 24 hours a day, 365 days a year, and the post holder may be required to work flexibly across shift patterns when required.

Please note:

  • New starters have a 6 month probationary period. This, together with the induction process aims to create a positive supportive working environment allowing new employees to learn key elements of their role over a reasonable timescale.
  • Candidates not currently employed by the Trust who attend an interview for non-qualified Band 1-5 posts are required to undertake numeracy and literacy assessments.
