Job summary

Oxford Health's Buckinghamshire Mental Health Directorate is working in collaboration with Buckinghamshire's Primary Care Networks (PCN) to develop and implement improved access to specialist mental health care and treatment within primary care services. Focusing on bridging the gap between primary and secondary care Mental Health services for individuals with complex and severe mental illness.

The service is designed to empower patients 18years+ to access specialistmental health support locally, within their GP Surgeries in a timely manner.

We are looking forautonomous, driven, and confident Mental Health Nurse Practitioner(s) who pride themselves on delivering ahigh-quality holistic approach for patients needing mental health support. Main duties of the job

The successful Mental Health Nurse will facilitate improved opportunities for joint working with our voluntary care sector, contributing to the leadership and development of national mental health agendas within their Primary Care Network (PCN). As part of the Primary Care Networks Multi-Disciplinary Team (MDT) your role will provide mental health triage, assessment and adviceto patients, carers, and colleagues. Facilitatingearly interventions toaid recoverywithin the community and utilising your profession specific skills to enable prompt access to specialist services for patients.

  • Independent and supplementary Prescriber
  • Recognised Post Graduate clinical or leadership qualification relevant to the role
  • Mentor and mentorship training
