The Adult Mental Health Team (AMHT) is excited to be recruiting Care Co-ordinators to join our busy but rewarding community team!
The City AMHT supports adults of working age (18-65) who experience a wide range of mental health conditions. We are committed to the person-centred recovery approach in all our clinical work and pride ourselves in offering a high-quality service reflected in our CQC rating of Good.You will join a dedicated staff group providing assessment and evidence based interventions to a wide range people in Oxford and the surrounding area.
We use the principles of a flexible assertive community treatment (FACT) model of healthcare. We have incorporated FACT into the daily functioning of the team; which enables us to be focused on the recovery of the patient through a multidisciplinary (MDT) approach; with expertise from doctors, social workers, nurses, occupational therapists, support workers and psychological therapies staff. We also have twice weekly MDT meetings, where we discuss clinical matters within our supportive team.
We strive to provide person-centred evidence based care to patients, where possible working collaboratively with their families and carers.
Main duties of the job
This role is open to registered mental health nurses who have extensive experience of working in adult mental health services and understand the challenges that this care group can bring.
You will be expected to deliver and maintain high quality and meaningful care for our patients. You will need to be a highly motivated practitioner who has experience of mental health services and can demonstrate the ability to initiate and manage your work effectively. The team is highly supportive and committed to delivering high quality patient care; where the caseload can be demanding, diverse and frequently complex.
You will need to be able to work autonomously, with a caseload of patients with whom you will engage and offer the recommended treatment and support. You will have experience of CPA process and able to assess and plan treatment with service users. You will also be able to assess and manage risk, including use of Safeguarding processes.
There are opportunities for team working and facilitating groups such as MBT/CBT based interventions. The MDT consists of medics, psychological therapists, social workers, OTs, RMNs and support workers, as well as embedded workers from the third sector to support with employment, housing and social needs. In addition we have embedded Peer Support Workers within the teams to promote the power of lived experience and ensure that the gap between patient and clinician is bridged.

The team are working using the FACT model of care. Caseloads for care coordinators range from 20 - 25 cases and are made up from a variety of presentations. Team members also participate in being duty worker about once a week. The service is open Monday to Friday 9:00 to 17:00 not including bank holidays.
The City and Northeast Oxon Adult Mental Health Team's main base is at the Warneford Hospital, a stones throw from the centre of Oxford and the dreamy spires. We provide care for the residents of Oxford, the local towns of Kidlington, Bicester and Woodstock, and the surrounding villages. The team is split into three Flexible Assertive Community Treatment (FACT) teams; Willow, Oak, and Aspen, which are aligned to local Primary Care Network patches.
