JOB PURPOSE

To provide high quality mental health rehabilitation to patients referred to or within Community Therapy Service (CTS).
To provide patient-centred holistic assessment and intervention to older adults that meet the referral criteria for the service.
To provide person-centred care which empowers patients to manage their conditions effectively and maximises their potential to reach their goals of self-management.
To provide mental health expertise and advice to other CTS practitioners to support rehabilitation goals.
To work closely with other healthcare professionals and services particularly Community Mental Health Team (CMHT) for older people and to provide seamless coordinated care and support, preventing hospital admissions and supporting discharge home.
Main duties of the job
The community therapy services in the North Oxfordshire are looking for an enthusiastic Occupational Therapist, Nurse or Physiotherapist with experience in Mental Health or someone with an interest in developing their skills to join our team.
We are looking for an AHP or nursing professional with a mental health or community services background, who is enthusiastic to develop their clinical practice within innovative and supportive multidisciplinary teams, treating patients with a variety of clinical conditions in their own homes.
Oxford Health NHS Foundation Trust is an accelerator site for the Ageing Well national agenda, as part of the NHS Long Term Plan. This offers the opportunity to shape and develop both rapid responsive patient care and supporting patients to rehab in their own home environment. We are looking for someone who will be supported to develop their skills in both physical and mental health rehabilitation, in order to provide excellent, holistic care as part of a community MDT.
The Community Therapy Service aims to provide high quality integrated therapy services working as an integral member of the Neighbourhood Team. We work closely with other multidisciplinary team members, social services and voluntary organisations for adults with a wide range of physical and mental health conditions.

DUTIES AND RESPONSIBILITIES
Clinical Care
- To deliver a safe and effective rehabilitative patient care and discharge, working in partnership with other healthcare professionals and services.
- To provide holistic physical and mental health rehabilitation.
- To work flexibly across localities and at weekends to ensure the requirements of the service contract are met.
- To work autonomously as part of an interdisciplinary team to prioritise, assess, plan and implement patient specific programmes of care/rehabilitation, analysing patient needs and making judgements about best practice contributing to the continuous assessment of patients.
- To undertake complex mental health assessment and determine appropriate care plans utilising clinical reasoning skills, knowledge of evidence-based practice and professional expertise.
- When required undertake physical care assessment and treatment within scope of own professional practice.
- To participate in the service duty desk as required.
- To work collaboratively with the CMHT to minimise duplication, maximise efficiency and facilitate a seamless pathway between the two services for the patient understanding the roles and responsibilities of the clinicians for establishing and delivering a care plan for the patient.
- To work in partnership with patients to develop patient-centred goals ensuring valid informed consent whilst working within the legal framework with patients who lack capacity.
- To communicate effectively with all patients, and their families/carers using an advanced range of verbal and non-verbal communication tools ensuring their understanding of the diagnosis and care plan which may be complex and sensitive.
- To provide evidence-based education and training to patients and carers to empower them to self-manage their condition.
