Job summary

We are looking for a highly motivated, resilient, and flexible individual with a passion for improving the lives of young people. The post requires developing key relationships with community providers, and match families and young people's needs and interests to the right people and services, as well as support them to make this a success.

The team works across the local system with families, to ensure children and young people with the most complex needs, initially those at risk of admission or in hospital, receive appropriate personalised, integrated support. The post holder will work in partnership with case managers to find meaningful activities in the community for young people to engage and enjoy, so that they are able to stay (and thrive) in the community they know.

Whilst previous experience is desirable, we are looking for someone with the correct core skills for the role, and therefore a comprehensive induction and training will be provided.

Main duties of the job

Across Buckinghamshire CAMHS there are a number of young adults who reach the end of their clinical care within CAMHS without a need for a transition to adult mental health services, but who may benefit from third sector or additional universal service support in relation to wider social needs or difficulties. Currently the signposting for these people and their families is dependent on individual clinician knowledge of wider local services.This exciting role is intended to centralise knowledge of this alongside similar posts in adult services and enable more consistent access to this signposting for young adults and families leaving CAMHS. The social prescribe will support an ending of CAMHS provision that includes a holistic consideration of ongoing needs by connecting people to community groups and statutory services for practical and emotional support, identifying individual interests and signposting accordingly.

Job responsibilities

The post holder will be given full induction, including the ability to undertake key worker specific and recovery star training, as well as join a wide community of practice network.

The service actively encourages professional development, and retain a strong ethos of supporting staff well being.

The Social Prescriber will support primary clinicians and care coordinators to develop their own knowledge of local provisions through indirect work/professional consultation.

They will: Improve the experience of children & young people reaching the end of their journey with CAMHS who have social or non-mental health specific needs that are not able to be met by an adult mental health transition Improve the experience of Parent/carers & families navigating the network of external/third-sector agencies Improve access to community-based opportunities for appropriate inclusion and occupation Maintain peer support relationships with other social prescribers and similar roles in CAMHS so that knowledge is shared.The Social Prescriber and Recovery Worker will work in collaboration with the primary clinician, may meet people on a one-to-one basis, making home visits or via digital appointments where appropriate within organisational policies and procedures. The role includes: To work under supervision with children, young people and families identified as having needs outside of the remit of adult mental health services who may benefit from wider social and/or third-sector support Following on from any clinical work already undertaken in CAMHS, to use social prescribing to help improve patient outcomes in a person-centred way, accessing the right help at the right time To be a trusted person who actively listens and understands the young people and families they are supporting to help navigate different services and facilitate visits, introductions, applications etc when appropriate To be responsible for keeping an up-to-date database of social prescribing opportunities in the Buckinghamshire area Promote this role within the CAMHS service, including joining pathway team meetings To support colleagues across the system to consider individuals wider social needs and develop knowledge of how to support access to services alongside CAMHS if needed at an earlier point in their care To actively participate in the gathering of service user, family/carers and other stakeholders feedback

  • All new starters have a 6 month probationary period. This, together with the induction process, aims to create a positive supportive working environment allowing new employees to learn key elements of their role over a reasonable timescale
  • Candidates not currently employed by the Trust who attend an interview for non-qualified Band 1-5 posts are required to undertake numeracy and literacy assessments.

Essential

  • A proven understanding of the complex needs (including mental health) and characteristics of children and young people with a diagnosis of mental health conditions
  • A proven understanding of strategies and resources that will support children and young people with mental health conditions
