Join our new team of doctors working to improve the Urgent Care and Community Care Pathways across Oxfordshire.
Oxford Health's Primary, Community & Dental Care Directorate is putting together a team of enthusiastic and innovative GPs and consultants to provide senior medical leadership for a wide range of primary and community care services across Oxfordshire.
The Trust's services already operate to a high standard, but we are launching an ambitious service transformation programme and recognise that the addition of more medical leadership will enable the team to provide even better care for patients and families.
To enable this, we are putting together a senior medical team to lead a number of our integrated care pathways. These pathways include a wide range of services in Oxfordshire, including (not exhaustively):
- District Nursing, community therapy and specialist nursing services
- Services for vulnerable groups and people experiencing health deprivation
- Urgent primary care, out-of-hours and minor injuries services (First Contact & Primary Care Pathway)
- Ambulatory and frailty care, hospital at home, end-of-life care and virtual wards
Main duties of the job
The Medical Pathway Lead is an important and responsible role in our senior Directorate team that will require you to oversee a pathway area containing a related suite of services. The role will require you to lead and implement change, working with senior clinical and operational colleagues to review, redesign, integrate and continually improve the overall care pathway, in partnership with health, social care and voluntary sector partners in Oxfordshire and BOB ICS (Buckinghamshire, Oxfordshire and Berkshire West Integrated Care System). Working closely with these partners, you will play a central role in steering how we provide care in the future, to better meet the needs of the population.
