Job summary

Are you passionate about improving functional outcomes and promoting quality of life for people requiring Rehabilitation within our Community Hospitals?

An exciting opportunity has arisen for an inpatient Band 5 Occupational Therapist to join our Multidisciplinary team at Witney Community Hospital.

You will be supported by a Senior Occupational Therapist and have the opportunity to consolidate your learning and focus on your core and specialist skills within Rehabilitation.

Main duties of the job

To deliver safe and effective rehabilitative patient care and discharge, working in partnership with colleagues in the multidisciplinary team within the community and acute sectors, supporting the efficient use of resources across the whole system.

Job responsibilities

To participate in the hands-on delivery of patient care.To be visible and accessible to patients, relatives and colleagues to work effectively within the team. To work under indirect supervision as part of an interdisciplinary team to prioritise, assess, plan and implement patient specific programmes of care/rehabilitation, contributing to the continuous assessment of patients.To work with multi-disciplinary teams to optimise care packages and performance ensuring appropriate and effective interagency liaison.To liaise frequently with your supervisor/senior regarding more complex assessment, clinical reasoning and proposed treatment plans.To involve patients in goals and treatment proposals gaining valid informed consent whilst working within the legal framework with patients who lack capacity for informed consent.To communicate effectively with all patients, and their families/carers using a range of verbal and non-verbal communication tools ensuring their understanding of the diagnosis and treatment plan.To accurately document and record all assessments within organisational and professional guidelines.To evaluate patient progress and alter treatment programmes seeking guidance as and when appropriate.To develop comprehensive discharge plans and work with relevant professionals to arrange ongoing care.To manage clinical risk within own patient caseload seeking support where required.To ensure patient dignity is enhanced throughout service delivery.
