The Trust is seeking to recruit an Adult of Working Age Consultant. The post holder will be based at The Neil Unit, Adult AMHT, Warneford Hospital site.
Come and join a team that is part of the well-established Community Mental Health service. These services form part of the patient pathway and work in conjunction with our inpatient mental health teams, crisis resolution and home treatment teams, as well as early intervention services.
Staff within the AMHTs work around Consultants and catchment areas that correspond as much as possible to specific geographical areas and GP surgeries, to allow maximum familiarity with these patients and better links with the corresponding GPs in their Primary Care Networks (PCN) The 5 AMHTs across the whole of Oxford Health NHS FT are based in Banbury, Oxford City and Wallingford in Oxfordshire and Aylesbury and High Wycombe in Buckinghamshire with satellite clinics based throughout the counties in all existing market towns, as per our current provision.
The AMHT's work closely with primary care, from where they receive most of their referrals. Teams triage, signpost and assess all new referrals, following assessment, these patients may be taken on by members of the AMHT for acute or longer-term treatment or signposted to other services or agencies as appropriate.
Main duties of the job
Following the integration of our existing Community Mental Health Teams (CMHTs), Crisis Service and Assertive Outreach Teams the newly formed locality based Adult Mental Health Teams (AMHTs) will undertake both an assessment and treatment function. This will therefore reduce the need for patients to be moved from one team to another especially when they are experiencing a crisis and are consequently very unwell. It will also enable patients to be familiar with a wider group of staff from the AMHT, so therapeutic relationships with individual patients and carers are maintained during a period of crisis for the patient, an inpatient admission or due to planned or unplanned leave of their nominated care coordinator. The consultants now provide combined function role having previously working with either the Assessment or Treatment teams within the AMHT.
The AMHT will work closely with the Short-Term Intervention Hub which offers online group-based psychotherapeutic interventions to patients under the AMHT. The AMHT also links with the Crisis Resolution and Home Treatment Team which runs alongside the AMHT and takes over patients requiring more intensive support in the community, often on discharge from hospital and/or to prevent admission; the CRHTT has its own medical staff.

The Consultant will provide senior medical input and clinical leadership to the team. Responsibility for the physical health care of patients usually remains with the GP. The post holder will have responsibility for those patients seen by himself/herself, together with those patients seen by medical staff directly supervised by the post holder. The post holder will work alongside the Service Lead, Team Leader and the other Consultant Psychiatrists in the team to ensure the effective functioning and development of the service. Contribution to developing and delivering improved clinical services as outlined in the National Service Framework for Mental Health and the Mental Health Long Term Plan. Involvement in Primary care network development and partnership working is an important part of this job.
Teaching and Training: The Oxford Health Foundation Trust is committed to high quality teaching and training for medical students, post graduate trainees in psychiatry, specialty doctors, and students and staff of other professions. The current college tutor is Dr Kathleen Kelly. Dr Alastair Reid is currently the Director of Medical Education. The consultant will provide direct clinical supervision and line management to the trainees, speciality doctors and any senior trainee placed with the team in agreement with consultant colleagues. This will include 1 hour per week clinical supervision for trainees working directly with them and an agreed amount of supervision, following the trust supervision policy, for specialty doctors. To be eligible for providing educational supervision, consultants will need to complete specific training and be approved by the Trust and HETV (Health Education Thames Valley). ST 4-6 trainer status can be sought after 1 year in a consultant post. Liaison with the Trusts postgraduate education lead and college tutor is expected. The consultant will be expected to participate in and attend the local training programme for junior doctors. Consultants should ensure that junior medical staff are working with the post holder to operate within the parameters of the New Deal and are Working Time Directive compliant. In addition, contributing to the multi-disciplinary development and training of staff of other professions who are working with the community and inpatient teams.

Oxford medicine was the highest rated undergraduate course in medicine in the National Student Surveys, and indeed the highest rated of any undergraduate course in any subject area in the UK, with 99% satisfaction. The Consultant appointed to this post will be expected to play an active role in teaching medical undergraduates and postgraduates. Postgraduate medical training is achieved through both didactic teaching in the Oxford University post graduate course and through educational supervision of trainees at foundation and specialist training (ST1-6) levels. The Consultant may also contribute, directly and indirectly, to the training of students and qualified staff in other mental health professions. The teaching of medical students includes direct teaching of students attached to the consultant team in community settings, and support for other members of the team who teach the students. Teaching of Oxford University medical students either placed in your team or through other avenues e.g., examining, lectures, ARCP is expected.
Providing timely written correspondence to relevant professionals documenting assessments, on-going management, progress, and eventual discharge using Trust IT and clinical records system training will be provided where needed. The consultant will be expected to provide direct clinical care and ensure medical involvement in the CPA process. Work collaboratively with other agencies, in particular liaising with staff from other teams to manage smooth and timely transfer into and out of the community team. The consultant will undertake some of the administrative duties associated with the care of patients.
Governance all services are expected to maintain effective governance including measuring and reporting performance. This is based on national standards and local contracts. Consultants provide a lead in the team in reviewing performance and governance. Key quality and outcome measures are embedded in the care packagesaligned to each care cluster. There will be an expectation to contribute to the collection of data as required by the Trust and other relevant agencies. This includes the timely recording of clinical activity data, and participation in clinical audit with appropriate administrative support. The Oxford healthcare improvement team supports services to ensure that quality improvement is embedded in what they do. Consultants are encouraged and supported to participate in service evaluation and development.
Rotas: Participation in the Oxfordshire Adult Consultant Out of Hours Rota: on call is currently approximately 1 in 28 Category A 3%. There are usually two specialist registrars ST4 to ST6 trainees, on call, who take first line responsibility for referrals from the community. The ST4 or ST6 trainees also provide second line support to ST1 to ST3 trainees on shift duty at the main hospital sites. The Warneford Reception is the contact centre for out of hours on call issues. The adult on call rota covers responsibility for all adult wards in Oxfordshire in addition to covering the community adult service, CRHT, Perinatal, EIS, EDPS, OUH wards and community Learning Disability service. Most work is done remotely via telephone but consultants are expected to follow the seclusion and long-term segregation policies and contribute to clinical reviews where needed out of hours and follow the current out of hours process at weekends or bank holidays by joining the senior leads telephone calls each day at 10am. If a consultant is unable to be on call, out of hours, due to sickness then they should inform Warneford reception who will follow a trust process for internal consultant cover. IF a consultant is aware prior to their OOH on call that they cannot attend due to sickness they should inform their line manager. Internal locum hourly rates are provided for planned cover of vacant shifts. There is also a daytime no Fixed Abode rota (NFA) which all consultants are rostered on for newly presented patients who have no fixed address in Oxfordshire who require an assessment at either Abingdon or Banbury Police stations. Currently the number of cases within hours for this work are approximately 10 per year.
Continuing professional development (CPD) The Trust expects all consultants to engage in Continuing Professional Development (CPD), for which one notional programmed activity per week is allocated. Continuing professional development is required of all consultants and supported by the Trust including access to study leave (up to 30 days over 3 years) and expenses (currently £600 per annum or £1,800 over 3 years). He/she will maintain ongoing professional development through active membership of a PDP group. There is a monthly clinical consultant peer supervision group. It is expected that the post holder will be in good standing with the college for CPD. Use of SPA time for research and special interest sessions are encouraged. There is an expectation to attend and participate in the academic programme of the Trust, including lectures and seminars as part of the internal CPD programme.
Mental Health Act and Responsible Clinician Approval: The post holder would be expected to take on the role of Responsible Clinician or be willing to undertake training to obtain Section 12(2) and Approved clinician training and will be expected to renew this approval according to agreed procedures.
