Job summary

Are you a Nurse (RGN, RMN or LDN) or Social Worker looking for your next challenge?

We are looking for a dynamic person, a highly motivated practitioner with excellent assessment and communication skills. You will be able to demonstrate a wider breadth of knowledge in relation to case managing adults with complex long-term conditions that originate from both mental health and physical health co-morbidities.

Main duties of the job

Key aspects of the role include;

  • Proactively case manage the most complex patients that are eligible for continuing healthcare to ensure their risk of further health deterioration is minimised.
  • Work collaboratively with individuals, providers and other members of the MDT to ensure that individuals needs are appropriately met.
  • Chair MDT Review Meetings.
  • Undertake Mental Capacity Assessments
  • Clinically lead on safeguarding concerns and attend any Strategy Meetings as required.
  • Support the CHC service to ensure that the legal framework with regards to Deprivation of Liberty Safeguards are met as required.
  • Act as a clinical matter expert for the team
  • Produce large, detailed written reports and therefore you will need to be proficient in the use of Microsoft Word.

About us

NHS Continuing Healthcare (CHC) means a package of ongoing care that is arranged and funded solely by NHS where the individual has been assessed and found to have a 'primary health need' as set out in the National Framework National Framework for NHS Continuing Healthcare and NHS funded Nursing Care.

We are a friendly and welcoming team. You will be allocated a mentor and will be fully supported through your induction programme. By the end of your first year you will achieve the following key skills;

  • In-depth knowledge of the National Framework and key supporting legalisation such as Mental Capacity Act
  • Experience in delivering the in-house rolling training programme for CHC
  • Experience in writing Witness statements to support DOLs appeals
  • Chairing complex multi-professional MDT meeting
  • Panel membership in relation to decision making panel regarding CHC eligibility including appeals
  • An understanding of integrated commissioning

You will need to be a car driver as assessments can take place in the Acute sector, clients homes, Community Hospitals and Hospices across Buckinghamshire and out of area where required. If necessary, adjustments can be considered in line with the Equality Act 2010. There is an opportunity to work flexibly to meet the needs of the service and this includes some working from home.
