Job summary

Oxford Health's Buckinghamshire Mental Health Directorate is working in collaboration with Buckinghamshire's Primary Care Networks (PCN) to develop and implement improved access to specialist mental health care and treatment within primary care services. Focusing onbridging the gapbetween primary and secondary care Mental Health services for individuals withcomplex and severe mental illness.

The service is designed toempower patients 18years+to access specialistmental health support locally, within their GP Surgeries in a timely manner.

Main duties of the job

The successful Social Worker will facilitate improved opportunities for joint workingwith our voluntary care sector, contributing to theleadershipand development of national mental health agendas within their Primary Care Network (PCN). As part of the Primary Care Networks Multi-Disciplinary Team (MDT) your role will provide mental healthtriage, assessment and adviceto patients, carers, and colleagues. Facilitatingearly interventionstoaid recoverywithin the community and utilising your profession specific skills toenable prompt access to specialist servicesfor patients.

As the Social Worker in this post, you will be expected to use your profession specific skills and values in this role and apply Social Work theories to practice, such as rights and strengths based approach, alongside your expert knowledge of the Care Act eligibility and safeguarding principles to support your clinical practice and signposting to appropriate services.

Desirable

  • Recognised Post Graduate clinical or leadership qualification relevant to the role
  • Practice Educator/ AMHP/ Best Interest Assessor
  • Assessed Supported Year in Employment/ or Equivalent
