A new exciting opportunity has arisen for an experienced clinical manager to lead the implementation of personalised stratified follow-up and remote monitoring for cancer patients deemed eligible for self-management, supporting individuals through their follow-up pathway, ensuring a safe effective service
The post holder will be required to operationally lead a small team responsible for the digital management of Personalised Stratified Follow-up (PSFU) using a digital remote monitoring solution.
The post holder will ensure that patients referred to Remote Monitoring, follow the correct pathway, taking responsibility for coordinating activity schedules including blood tests/imaging and reporting outcomes to the patient, their GP and clinical teams
We would welcome applications from individuals educated to degree level with evidence of continual role development
Interested applicants must have a minimum of three years' experience in an administrative role. Have extensive IT skills and in-depth knowledge of the Somerset Cancer Registry and a proven track record of monitoring healthcare targets. Knowledge of the national personalised care agenda is essential along with experience of multi professional working and managing other staff.
Good communication, interpersonal and organisational skills are a key component of the role along with the ability to analysis data and produce reports
Main duties of the job

Job responsibilities
To implement a Trust wide approach to remotely monitor patients deemed suitable for supportive self-management in line with local, regional and national guidelines, acting as the operational lead for a central service.
To be responsible for the day to day running of the digital Remote Monitoring Service, ensuring all patients stratified into supportive self-management are registered onto the system and monitored in line with agreed protocols.
To be familiar with tumour site specific pathways, entry points into Remote Monitoring, risk classification and discharge protocols
To act as the line manger for the Personalised Care Assistant
To register patients onto the remote monitoring system, allocating the correct risk pathway and activity in the absence of the Personalised Care Assistant
To establish and maintain effective working relationships with the Cancer Clinical Nurse Specialists and other members of the MDT
To work with clinical teams to agree the parameter for Remote Monitoring per tumour site.
To act as a point of contact for patients who have questions/concerns related to their Remote Monitoring schedule/tests/investigations
To develop letter templates as part of the communication process with patients/GPs entered onto the supportive self-management programme, ensuring these are generated at the appropriate point in the pathway
To inform patients and their GP by letter of test results that are within clinically agreed ranges
To identify test results outside the expected range, communicating with Cancer Clinical Nurse Specialist in a timely manner to ensure prompt assessment and review by the clinical team
To identify and contact patients who do not engage in their follow-up plan ascertaining reasons why and escalating concerns to the Cancer Clinical Nurse Specialist on a weekly basis
To produce weekly reports on Remote Monitoring activity for each tumour site and the Macmillan Personalised Care Lead
To assist in the planning, development, monitoring and review of cancer pathways to embed the key principles of stratified follow-up
To be responsible for testing upgrades to the Remote Monitoring System on the Somerset Cancer Registry, communicating changes to clinical/administrative teams, acting as a point of contact for queries/concerns
To train staff on the use of Remote Monitoring, ensuring changes in practice are communicated to the relevant teams
To promote a culture of innovation and change to meet the needs of the service.
To facilitate the introduction of modern technology within the team and responsible for escalating Technical/IT issues
To ensure standardisation of administrative functions, related to personalised care across the organisation.
To participate in relevant research/audit to assist with learning and service development
