Northamptonshire Community Stroke Team
The Northamptonshire Community Stroke Team (N-CST) has delivered a robust Integrated Community Stroke Service model (ICSS), as per the national guidelines, for over a decade. N-CST continues to see patients within 48 hours of discharge home, for rehabilitation assessment and intensive goal-based rehabilitation. The patients time with the team is needs based and patients can access any of the other services they require, regardless of time post stroke. Rehabilitation is completed by a stroke specialist MDT, that includes Physiotherapists, Occupational Therapists, Speech and Language Therapists, Nurses, Rehabilitation Practitioners, Rehabilitation Assistants and Admin staff. The team also has face to face exercise groups, remote groups and offers six-month reviews.
Main duties of the job
Responsible for providing and co-ordinating physiotherapy assessment and treatment for a caseload of stroke patients in the NGH Community Stroke Team
Responsible with the MDT for managing and co-ordinating treatment, care and rehabilitation for patients on the caseload in conjunction with the patient, family and carers within the patient's own home or place of care
Supervise, appraise and develop band 5 physiotherapists, band 4 and 3 staff and student physiotherapists in the team
Support band 7 and 8 staff in managing and planning the stroke rehabilitation services, deputising for Physiotherapy team lead as required.
To take a role in the specialist assessment and treatment of stroke patients who may have complex presentation, to determine the physiotherapeutic interventions indicated to provide the best outcome for the individual using evidence-based practice and establish clinical reasoning and knowledge where recommendations may not agree.
To utilise and further develop specialised skills for stroke, including therapeutic handling (e.g. ability to move complex patients from bed to chair).
To identify organisational changes within NGH and external influences which will effect NGH stroke services and act accordingly.
To record information that has been personally generated following assessment and treatments.
Build safe working knowledge of all equipment used to provide patient care and participate in risk assessment, maintenance and training.
