Job responsibilities
Clinical CareTo work in partnership with the cancer multidisciplinary teams to develop, implement and evaluate a specialist nursing service for young people aged 13-24 years diagnosed with cancer across Northamptonshire.
To work in collaboration with the tumour site MDTs and paediatric oncology and haematology services, ensuring all patients between 13-24 years receive age-appropriate high quality patient centred care in line with national directives.
To establish processes to ensure patients (13-18 years) are referred to the Principal TreatmentCentre.
To ensure patients (19-24years) are supported to make informed decisions regarding place of care, either locally within the designated hospital or referred to the Principal Treatment Centre.
To work closely with the TYA MDT Coordinator, Paediatrics and site-specific teams to ensure all newly diagnosed TYA patients are discussed at the TYA MDT.
To provide comprehensive highly specialist, clinical care, information and support to teenagers and young adults living with cancer, acting as the keyworker where appropriate in the absence of the tumour site nurse specialist.
To develop effective communication with patients/carers utilising different media including face to face contact, text, emails and home visits.
To assess the holistic needs of young people diagnosed with cancer, responsible and accountable for the delivery of safe effective care.
To support the completion of end of treatment summaries and care plans, working with the Macmillan Personalised Care Transformation Lead.
Support the development of patient focussed education, including information to self-manage ongoing consequences of treatment.
To identify and meet the specialist information needs of patients/carers utilising local/regional and national resources.
To provide Level 2 psychological support for patients/carers, referring patient with complex needs to the Macmillan Clinical Psychologist.
Use professional judgement to intercede and act as an advocate for patients to ensure a person focused approach to the delivery of care and service strategy.
Support and enable patients/carers to make informed decisions relating to their treatment and care, escalating concerns to the relevant team.
Participate in the TYA multidisciplinary meetings at the PTC, ensuring all newly diagnosed TYA patients are referred to the MDT in a timely manner, contributing to the discussion and patient assessment/care planning decisions.
Ensure discussions at the TYA MDT are communicated to the local team to ensure continuity of care and a seamless service for patients.
Support transition from childrens cancer services to TYA and into adult services.
Develop effective working relationships and pathways to enable appropriate referral of TYA patients to other professionals for more specialist support e.g., palliative care, clinical psychology, social care and youth support coordinator.
Liaise with community services to ensure continuity of care and the provision of seamless care delivery.
Act as a facilitator regarding appropriate recruitment into clinical trials/research programmes.
To maintain accurate clinical records documenting in the hospital notes and on the Somerset Cancer Database.
Professional/Leadership Responsibilities
To adhere to the NMC Code of Professional Conduct and other national regulations.
To identity own personal development needs to work at specialist level and take appropriate action to ensure that these are met.
To maintain a personal professional profile, demonstrating clear evidence for nursing revalidation.
Work closely with the multidisciplinary teams locally and at the Principal Treatment Centre (PTC) to ensure alignment of goals and priorities between national and local modernisation initiatives relative to TYA services.
To act as a specialist resource to healthcare professionals relating to TYA cancer patients between the ages of 13-24 years, providing expert advice and support to improve clinical outcomes and patient experience.
To provide and receive highly complex and sensitive information, negotiate, influence and act with diplomacy at a high level.
To work with the TYA Lead Nurse and key stake holders on the development of clinical practice, standards of care including the development of policies, procedures, protocols and guidelines to enhance patient care.
To influence, lead and implement change in clinical practice across the County in collaboration with the Teenage Cancer Trust Lead Nurse to improve the experience of those living with cancer.
Educate and support health and social care colleagues so they better understand the needs of teenagers and young people living with cancer.
To ensure the TYA service is developed and implemented in line with National directives related to cancer.
To promote the philosophy, values and ethos of the Teenager Cancer Trust Charity.
To promote collaborative working relationships and effective communication between the Trusts, the Principal Treatment Centre and the TYA Lead Nurse.
To act as a role model demonstrating high standards of holistic care, providing clinical leadership in TYA cancer.
To attend Teenage Cancer Trust events and meetings as requested and agreed with the charity.
To participate in the National Teenage Cancer Trust funded nurses group for mentorship and development.
Contribute to the delivery of the Teenage Cancer Trust Nursing Outcomes and formulate an annual report for cancer services and the charity.
Education
Work with the Teenage Cancer Trust TYA Lead Nurse to contribute to the delivery of local and national education programmes for multi-professional staff that have direct or indirect contact with TYA cancer patients.
Develop, deliver and facilitate training and education programmes for other members of the cancermultidisciplinary team across the County.
Attend national, regional and local meetings/conferences as appropriate and disseminate information to the team.
Develop and work in accordance with the Trusts clinical governance framework.
Audit/Research
To collect activity data to demonstrate the effectiveness of the specialist nursing service.
To actively seek feedback from patients/families to inform service developments.
To evaluate the effectiveness of the TYA service, implementing change to enhance the patient/family experience.
Participate in local/regional/national research initiatives as appropriate.
Undertake agreed audit programmes specific to the provision of TYA cancer.
Participate in external and internal projects/audits evaluating various aspects of clinical care.
Contribute to and implement clinical guidelines, review and update them on an annual basis in agreement with the multi-disciplinary team.
Teenage Cancer Trust Commitments
Contribute to the implementation of the strategic objectives of Teenage Cancer Trust.
Convey information/data on activity and outcomes of this role to the Teenage Cancer Trust in line with the charitys reporting requirements.
Communicate with Teenager Cancer Trust on service commissioning, development and delivery at a regional level.
Attend Teenage Cancer Trust events and meetings as requested and agreed with the charity.
To participate in the national Teenage Cancer Trust funded nurses group for mentorship and development.
