Job summary

This is an exciting time to join our Elective Surgical Admissions team as we roll out Dedicated Ward Pharmacy (DWP) across the whole trust. Your focus will be leading and developing the pharmacy service within POAC, where we already have had established presence.

Main duties of the job

You will be part of a pharmacy team that is responsible for the pharmaceutical care of our elective patients throughout their whole perioperative journey. From reviewing patients in POAC, where you will liaise with anaesthetists, surgeons and other MDT members to optimise their pharmaceutical management. To assessing patients in MTAU on the day of surgery. Then following up patients on our surgical wards.

This role extends to providing advice and educating medical and nursing staff, as well as contributing to the development and implementation of medicines management policies and procedures.

You will be working within our established surgical pharmacy team. There will be opportunities for working on all the surgical wards and our new Critical Care unit. You may attend consultant led ward rounds, including our Acute Pain Team and Nutrition Team as well as other areas within the surgical division which will provide plenty of opportunities for learning and CPD.

You will be fully supported and encouraged in developing your clinical qualifications further including independent prescribing if required. This is a full-time post but substantially part-time may be considered.

Clinical Services

To proactively promote safe and evidence-based medicines related advice with all grades of doctors, prescribers, and nursing staff within POAC by communicating and providing highly complex information on medication and challenging poor practice.

To prescribe safely and in line with the formulary, policy, procedures or guidance.

To provide general and specialist pharmaceutical advice and support to POAC staff, to enhance the quality of patient care which will include all patient aged groups and of many morbidities.

To liaise closely with medical, pharmacy and nursing staff to lead, develop and implement complex therapeutic guidelines, standard operating procedures and medicines-related policies in POAC, so as to provide advice where information is lacking or medical opinions may differ.

To manage and participate in the delivery and development of the Clinical Pharmacy Service eg, proactive participation in ward rounds, input into medicines management review, acting as a clinical pharmacy role model being accountable for your own professional actions.

To develop standards of clinical pharmacy practice which align with national frameworks and performance management measures with Principal Pharmacists.

Maintain an up-to-date clinical expert knowledge in clinical pharmacy and pharmacy practice to ensure a broad base of pharmaceutical knowledge and an advanced level of clinical reasoning and judgement. To actively share this knowledge with the wider Pharmacy team

To optimise medicines management by reconciling drug histories, performing medication reviews and using patients own medicines.

To detect, record and report adverse drug reactions identified to the Medicines and Healthcare Products Regulatory Agency (MHRA) and via Datix as per Trust Policy.

To record your clinical activity and to monitor the outcome of your advice.

To fulfil the requirements of the Standards for Pharmacy Professionals.

To implement the Duty of Candour.

Medicines Management

To ensure that the principles of safe and secure handling of medicines andcontrolled stationery are adopted and to take part in the quality assurance systems for pharmaceutical services to comply with legislation and Quality Standards eg, Care Quality Commission.

To participate in preparing business cases for the Directorate when they involve medicines.

To demonstrate awareness and commitment to the Trusts Clinical Governance agenda.

To implement, manage and co-ordinate the introduction of ePMA within POAC and Surgery Division in liaison with the ePMA team, prescribers and nursing staff.

To participate in the analysis, identification and management of medication risks.

To lead on the development of guidelines for the use of specialist medicines in POACTo manage the introduction of new medicines by undertaking formularysubmissions, evaluating the clinical and financial impact in secondary and primary care and identifying risks associated with new medicines.

To ensure the provision of specialist information on the use of medicines for the on-call Pharmacists out-of-hours.

To liaise with the Surgical Pharmacists to manage the pharmaceutical aspects of the care of elective surgical patients

To ensure that clinical and non-clinical incidents are reported, recorded,investigated, analysed and appropriate action taken and/or proposed.

Financial

To be responsible for the co-ordinated advice to the directorate to effectively manage expenditure on medicines, including tariff medicines, excluded medicines, prior approvals, individual funding requests etc.

To horizon scan for new medicines and to advise on clinical and budgetary impact.

To produce appropriate reports documenting the use and expenditure of medicines to evaluate trends and variations in prescribing in order to develop and implement policies aimed at containing expenditure and realising savings.

To identify and manage medicine/pharmacy Quality/Innovation/Productivity/Prevention (QIPP) Schemes, including Cost Improvement Programme (CIP) Schemes within the directorate to ensure this is coordinated within the Pharmacy and Trust.

To be an authorised signatory for Pharmacy to commit expenditure via the Trusts Standing Financial Instructions and delegated ordering system.

To provide expert professional advice on medicines to clinicians, managers, finance and commissioners that may affect POAC or Surgical Directorates.

Education, Training, Staff Management and Supervision

To act as a role model, supervisor and mentor for Senior Clinical Pharmacists, Rotational Pharmacists, Trainee Pharmacists, Medicines Management Technicians and other members of Pharmacy staff.

To line manage and undertake appraisal for Clinical or Specialist Pharmacists and have responsibility for their on-going learning, development and appraisal.
