Job summary
We are looking for an enthusiastic and enterprising candidate to join a progressive and forward thinking department of 12 consultants, each with their own special interest. We have well-developed services in oncology, minimal access surgery, infertility, colposcopy & out-patient hysteroscopy, high risk obstetrics and Urogynaecology services. This post is a replacement following a colleague relocating.
In order to meet RCOG requirements for direct consultant involvement on the labour ward, the appointee will take part in a 1:8 rota alternating between Obstetrics and Emergency Gynaecology. On a 1 in 8 weekly rotation all the Consultants drop any other planned commitments, both clinical and non-clinical, and provide obstetric or emergency gynaecology cover during the daytime. The Consultant covering Emergency Gynaecology will also be responsible for overseeing the Elective Caesarean section list. The Obstetric Consultant will provide cover for both Obstetrics & Gynaecology at Weekends.
This rota provides 66 hours of prospective Consultant labour ward cover per week in line with Maternity Matters, NHSLA and RCOG requirements and the new Ockenden recommendation. The Job will be paid at 10 PA's
Main duties of the job
The candidate should be keen to work as part of a team, continuing to develop good relationships with other staff within and outside the Clinical Management Team. The appointee will be expected to live within 30 minutes travelling time from the hospital.
In the last few years there has been considerable site development at Kettering General Hospital. In particular a new theatre block for Obstetrics & Gynaecology was constructed and opened in February 2016.
There is a thriving and well-equipped Post Graduate Education centre including a new, large, modern library, seminar rooms, and lecture theatre.
The Hospital has a state of the art 16 bed ITU/HDU. There is a two bed Enhanced Care facility on Labour Ward where patients are managed with multidisciplinary input from obstetricians, anaesthetists, and physicians.
There are fully supporting Pathology and Radiology services. Radiology provides ultrasound, nuclear medicine, MRI and computerised axial tomography on site. There is a well-equipped obstetric ultrasound department in the Maternity Unit as well as a specialised Fetal Medicine Service provided by visiting Consultants from University Hospital of Leicester.

Because of the geography of the area the Trust has peripheral clinics in several areas. In addition to those at Kettering there are clinics run at Corby Diagnostic Centre and in Nene Park Outpatients Clinic. Most peripheral clinics are combined Obstetric and Gynaecology and have varying facilities, including phlebotomy, X-Ray and ultrasound.
GYNAECOLOGY
There is a 12-bedded gynaecology ward which manages elective surgical and emergency admissions.
There is a 4 -bedded day case ward that accommodates patients undergoing medical termination of pregnancy. Surgical terminations of pregnancy are performed on site.
We have an emergency assessment unit where patients with early pregnancy or acute gynaecological problems are assessed.
ANTENATAL SCREENING
We offer a full range of prenatal screening services including early dating scans, Nuchal Translucency and biochemical screening. We offer CVS as well as Amniocentesis. The Fetal Medicine Service is supported by Consultants from University Hospital of Leicester (attending 2 clinics per week). A Consultant Clinical Geneticist who covers the county supports the genetic services. There are on site prenatal counselling facilities with a specialist midwife and the service is offered to all women as part of their routine antenatal care.
THEATRES
There are two large and fully equipped state of the art theatres opened in February 2016. One is for routine gynaecology and the second theatre for Obstetrics. A third theatre is used for emergencies for obstetrics and gynaecology.
OUTPATIENTS
There is a modern outpatient clinic area with a minor operations suite and colposcopy service. In addition to general clinics there are several specialised clinics:

- Gynaecological Oncology Clinic
- Obstetric diabetic clinic
- One Stop Hysteroscopy clinic
- Urodynamics Clinic
- Pre-operative Assessment clinics
- Medical terminations

We have a well developed cancer unit, in the Centenary Wing, at Kettering and majority of gynaecology chemotherapy is administered here. Open multidisciplinary Gynae Oncology meetings video-linked to the cancer centre in Northampton are held weekly.
TEACHING
We are part of the East Midlands Deanery. There are separate ST1/ST2 and ST3 upwards training programmes. The other fixed teaching sessions are:
- Tuesday morning Gynae Oncology MDT meeting
- Friday morning CTG meeting
- Friday day teaching session including perinatal morbidity/mortality meeting and gynaecology morbidity meeting
Audit
Audit is actively encouraged at all levels. All junior staff are given an audit project each; six monthly for SHOs and annually for Registrars.
Office and Secretarial Support
Office space and secretarial support will be in place for this post.
Administration
The appointee automatically becomes a member of the local Medical Advisory Committee. The candidate will be expected to share in the general administration and management functions of the department.
Clinical Commitment
Shifting of clinical commitment from junior doctors to nursing staff has been developed actively in Kettering to allow more time for training and adaptability as further changes are imposed on us by reduction of Junior Doctors hours. We have specialist nurses in Gynae Oncology, Infertility, Termination of Pregnancy and Prenatal Diagnosis who all contribute to running the department.
ON CALL
The on call will be 1 in 8 with prospective cover. The On call week will start at 8am on the Friday and will include Saturday and Sunday sessions in the department from 9am to 13pm and then evening ward rounds (20:30-22:30,) and Monday to Thursday 8am to 6pm. On Mondays to Thursdays the Night On call will start with hand-over at 5-6pm and be resident for one hour until 7pm. On call will finish at 8am next morning. The weekends on call will be attached to the start of the obstetrics emergency week on call mentioned on page one of the job description. It is usually possible to swap weekend on calls to improve work life balance without affecting clinical commitments. In order to inform clinic and theatre adjustments, Mr Waseem Osman makes the distribution of the on call week well in advance, taking into account annual leave negotiations.
