We're looking for a skilled and motivated Mental Health Practitioner to join our dynamic team within the Sunderland and South Tyneside Access CBU.

What We're Looking For:

  • Registered professional (RMN, Social Worker, OT) with current registration.
  • Experience of children's and young person's and/or crisis services
  • Strong assessment, risk management, and care planning skills.
  • A collaborative approach and commitment to trauma-informed care.

What You'll Be Doing:

  • Delivering high-quality mental health assessments and interventions.
  • Supporting service users in crisis and facilitating safe, person-centred care.
  • Contributing to service development and quality improvement initiatives.
  • Working closely with colleagues across primary and secondary care pathways.

Main duties of the job

  • Crisis Intervention: Delivering short-term, intensive support to help stabilise individuals in crisis and reduce immediate risk.
  • Risk Management: Evaluating and managing risks related to self-harm, suicide, or harm to others, and implementing safety plans.
  • Multi-disciplinary Working: Collaborating with psychiatrists, social workers, and other professionals to coordinate care.
  • Care Planning: Developing and reviewing care plans tailored to the individual's needs, including referrals to appropriate services.
  • Documentation: Maintaining accurate and timely clinical records in line with legal and professional standards.

The successful candidate will be expected to exercise clinical judgment and contribute actively to the team.

Essential

  • First Level Registration RMN/RNMH/RNLD Qualification
  • Experience of working with service users and carers in successful delivery of treatment plans
  • Complete preceptorship programme for newly qualified nurses
  • Multi-disciplinary team work.
  • Proven ability to develop therapeutic partnership with service users and carers.
  • Proven ability to be able to communicate interventions to service users.

Desirable

  • Further relevant post qualifying training

Knowledge and Experience

Essential

  • Knowledge of relevant contemporary professional theories of mental health care and evidence based interventions.
  • Understanding of the statutory issues which impact on service users.

  • Previous experience in a community setting.
