Job summary

Are you an experienced and dedicated Registered Nurse with expertise in learning disabilities and mental health? We are seeking a highly motivated Band 7 Learning Disability Specialist Nurse to join our dynamic Learning Disability Community Treatment Team. This post is for 30 hours per week. This is an exciting opportunity to contribute to the provision of high-quality, specialist care for individuals with learning disabilities who also present with complex mental health needs.

In this pivotal role, you will act as a clinical leader and expert resource, working collaboratively across pathways to ensure the delivery of person-centred, safe, and effective care. You will have key responsibility for leading service development initiatives, facilitating and chairing complex risk forums, and conducting comprehensive, complex risk assessments to inform care planning and management. Your leadership in these forums will be instrumental in ensuring robust multi-disciplinary decision-making and safeguarding practices.

Your contribution will be essential in improving clinical outcomes, reducing health inequalities, and promoting autonomy and quality of life for individuals with complex and diverse needs.

Main duties of the job

The Specialist Nurse provides high-quality, compassionate care to adults with complex needs, working autonomously within Trust guidelines. They deliver advanced nursing interventions, manage complex risks, and support multidisciplinary teams. The role includes facilitating access to primary care, addressing health inequalities, and ensuring holistic responses to dual diagnoses. The nurse leads on clinical audits, promotes best practices, and contributes to staff training, supervision, and service development. Collaborating with Clinical Leads and Pathway Managers, they support performance and project planning to improve service quality, ensuring care is empathetic, respectful, and centred on recovery, wellbeing, and independence.
