Job summary
There is an exciting opportunity for a clinician to join the South Tyneside Psychiatric Liaison Team (ST PLT) based at South Tyneside District Hospital. We are a well-established team that provides care to people of all ages within a 24 hour service across 7 days.
ST PLT provide responsive assessment and intervention to people experiencing a range of mental health needs. We provide direct clinical care within the Emergency Department, inpatient wards and some outpatient clinics. We also support South Tyneside residents who are experiencing delirium within their own homes.
We strive towards clinical excellence and have accreditation with the Psychiatric Liaison Accreditation Network (PLAN). We work closely with CNTW colleagues and support seamless transitions of care across a multi-agency network.
We are looking for applications from proactive and dynamic candidates who are able to evidence autonomous decision making and excellent communication skills. Psychiatric Liaison Team clinicians require a comprehensive understanding of person centred assessment and complex risk formulation as well as in-depth knowledge about physical and mental health conditions.
ST PLT is an inclusive and supportive team with a visible leadership presence. We have a strong multidisciplinary approach consisting of pharmacy, psychology, occupational therapy, nursing and medical specialists.
Main duties of the job
Provide compassionate care that is based on empathy, kindness, respect and dignity.
Work as member of the multi-disciplinary team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.
Comprehensive biopsychosocial assessment of people experiencing crisis and/or symptoms related to mental health disorders including, but not limited to, anxiety, depression, psychosis and dementia.
Engage people and their carers in personalised risk formulation and safety planning.
To care plan and provide a variety of treatment including behavioural activation, solution focused, distress tolerance, cognitive stimulation and cognitive behavioural therapy based interventions.
Provide specialist clinical advice to colleagues within CNTW as well as professionals in the acute hospital, social care, police, ambulance and third sector services.
Provide clinical supervision to staff as delegated by pathway clinical lead and pathway manager.
