Job summary

East Memory and Later Life Services (MLLS) are excited to welcome an enthusiastic, compassionate and dedicated community practitioner to our team, to support in making a significant impact on the well-being and quality of life of older adults in Carlisle and Eden.

Our service are committed to providing outstanding care and support to older adults in our community who are experiencing memory issues, co-morbid functional mental illness and complex/age related needs. Main duties of the job

Our community practitioners work across three pathways: Early Memory and Wellbeing Pathway, Complex Needs Pathway, and CHESS pathway. Our Early Memory Pathway supports with the assessment of individuals experiencing concerns or decline in their memory and cognition, to support with establishing potential diagnosis and maximising quality of life.

Our Complex Needs Pathway provides initial nurse led assessments of patients with complex dementia, often with behaviour that challenges others, co-morbid functional mental illness and often with multiple physical health needs and or age related needs.Care Home Education Support Service (CHESS)- Support and education into a care home setting to help assess, formulate person centred interventions to help prevent hospital admissions and use of antipsychotic medication.

The community practitioner will have a pivotal role within the CHESS team, and will use the Newcastle Model (2017) to provide bespoke outreach interventions for care homes, alongside delivering an education package to help promote a person's independence and quality of life in a setting of their choice. We aim to avoid unnecessary hospital admissions and pharmacological interventions and opt for a more holistic, person-centred therapeutic approach with an array of interventions.

Person Specification

Eduction and qualification

Essential

  • Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
  • Mentor Training.
  • Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
  • Proven track record of post registration clinical experience relevant to the post.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
  • Up-to-date knowledge/experience of relevant legislation.

Desirable

  • Post Registration Qualifications in Psychological Interventions
  • Experience of working within a community team.
  • Non-Medical Prescribing
