Job summary
An exciting opportunity has arisen to work within the Transitional Discharge team. This team supports clients on In Patient wards at St Georges park , to overcome any barriers that may impact on them being discharged safely from hospital. You will work closely with inpatient wards and external partners in trying to resolve these barriers, ensuring positive outcomes for clients and their carers.
You may be asked to hold a small caseload or have responsibility for specific ward environment, under the supervision of an experienced qualified clinician.
Main duties of the job
To support clients on overcoming identified Barriers to discharge and assisting the Multi disciplinary team to facilitate safe and timely discharges from hospital.
To have responsibility for a group of clients or Ward under the supervision of a Qualified Nurse.
To Liaise and work closely with internal and external partners, Wards, OT's, CTT's, Local authority, Housing and DWP, to ensure any identified barriers to discharge are overcome in the best interest of the client.
To support families and carers.
To ensure clear positive communication with all stake holders.
To provide transitional support to clients leaving hospital that may require practical input, home visits, attend appointments, view properties, purchasing household items liaise with external companies with regard to utilities.
