Job summary

As a Band 6 Community Practitioner you will coordinate mental health care and treatment for patients within our locality area with cognitive and functionally frail conditions focusing on care in the community. You will provide thorough assessment and treatment plans within NICE guidelines to a caseload of patients across Sunderland. You will be able to work autonomously and as part of a team.

Our Older Adults Pathway is a multi-disciplinary team with other functions available which can support with complex patient needs such as our Day Hospital, Step Up Service, Behaviour Support Service, Physical health monitoring and Care home liaison team.

It will be expected that you will possess excellent communication, interpersonal skills and have an ability to manage your time effectively.

You will work in a highly supported environment and will be provided with training opportunities to develop your knowledge and skills. You must have a strong commitment to the delivery of high quality, safe, patient care. We are an Award Winning Team two years running

Our service is currently provided from Monday to Friday 9am - 5pm (excluding Bank Holidays) with some out of hour's flexibility 8a.m. - 8p.m. to meet individual need.

Main duties of the job

  • To be responsible for the assessment and management of casework of service users who have substantial and complex needs
  • In collaboration with the service user and carer where appropriate, develop formulation focused care plans utilising a psychosocial approach , ensuring timely reviews to monitor progress and adapt care plans as necessary
  • To provide compassionate care that is based on empathy, kindness, respect and dignity
  • To provide Clinical and Psychologically based interventions
  • Provide specialist advice to service users, carers, staff, statutory agencies, third sector partner agencies and the public
  • Provide clinical supervision to staff as delegated by Clinical Lead and Pathway Manager
