Job summary

An exciting opportunity has arisen for a Band 6 Community Practitioner to work in the North Tyneside Recovery Partnership (NTRP).

NTRP is a dedicated multifaceted service for any individual in North Tyneside who is 18 or over and is experiencing problems with drugs and/or alcohol. We believe recovery from addiction and substance misuse is possible for everyone, and we're here to help individuals achieve this. Our aim is to make recovery a reality in North Tyneside.

The team have many specialists and expertise within the partnership to aid out client group. We also have strong links with a number of services both within CNTW and externally and strive to work collaboratively to deliver high quality care.

The role entails working with those who are affected by substance and/or alcohol misuse.

The post holder will be expected to provide safe and effective interventions, harm reduction advice and deliver evidence based psycho-social interventions to support service users to consider and achieve recovery.

Main duties of the job

The post holder will be involved in assessment and formulation of service users to identify need for, and support onward referrals into other CNTW services and external agencies. There will also be a focus on developing the knowledge and skills within the service around neurodiversity, ensuring this is reflected within detailed formulations.

The team consists of a well-established mix of professionals that work collaboratively to deliver individualised care. The role involves individualised clinical assessment, care planning, formulation, and collaborative working across the multi-agency team.

The applicant will be expected to demonstrate leadership within the team, providing supervision and supporting students therefore is essential they have mentorship qualification.

To post holder will be expected to hold a specialist complex caseload.

To provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, 3rd sector partner agencies and the public.

In collaboration with the Service User and Carer where appropriate, develop Recovery and Wellbeing focused Care Plans, ensuring regular reviews to monitor progress and adapt Care Plan as necessary.

The post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.

Qualifications

Essential

  • Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
  • Mentor Training.
  • Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
  • Proven track record of post registration clinical experience relevant to the post.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
  • Up-to-date knowledge/experience of relevant legislation.

  • Ability to manage competing demands and prioritise workload
  • Experience of MDT working both internal and external to CNTW
