Job summary

The Newcastle and Gateshead Learning Disability Teams are happy to be offering an exciting opportunity for Band 6 Community Nurse Practitioner to work into our enhanced scaffolding provision.

The community teams are multidisciplinary. Working across the whole of Newcastle and Gateshead with individuals with complex needs, who are at risk of hospital admission, placement breakdown and presenting as acutely unwell. The successful post holder will be expected to demonstrate a strong commitment to high quality patient care, good organisational skills and be able to demonstrate prioritisation of workload. Key components of the role will be to ensure the service offer a safe, caring, responsive and well-led service.

The role will include liaising with agencies such as social care, service providers, inpatient services, safeguarding, alongside a range of other agencies. Within this team we pride ourselves on having a broad range of knowledge within these selected areas and ability to manage the risks associated with this.

Main duties of the job

Responsible for the assessment and management of a caseload of patients with substantial and complex needs, who are at risk of hospital admission, placement breakdown and presenting as acutely unwell.

Provide responsive and intensive specialist clinical advice to patients, families, carers, staff, statutory agencies, third sector partner agencies and the public.

Provide high quality assessment, a sound knowledge of the mental health act, risk management, safety planning, and care and treatment reviews, alongside offering the wider team support, advice and supervision.Work collaboratively with patients to develop care plans, safety plans and recovery and wellbeing plans. Ensuring patients have access to regular reviews to monitor progress and adapt plans as necessary.

Consider carer needs and ensure carers and family are involved in care planning

Deliver evidenced based clinical interventions.

The post holder will work as a member of the multi-disciplinary team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.

Coordinate treatment planning, interventions, clinical reviews and discharge planning.

Provide clinical supervision to staff as delegated by Team Manager and Clinical Leads

Provide compassionate care that is based on empathy, kindness, respect and dignity.

Incorporate a patient centred philosophy into nursing practice and patient centred collaborative care.

Education and qualification

Essential

  • Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
  • Mentor Training.
  • Evidence of post-registration training in clinical interventions/skills relevant to the post.
  • Proven track record of post registration clinical experience relevant to the post.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
  • Up-to-date knowledge/experience of relevant legislation.

Desirable
