Job summary

This is a unique opportunity to work as a GP to provide primary care level input to manage acute and chronic health conditions, supported by a physical health team, for patients on Northgate Park who can have a long stay in hospital. You shall be an integral part of the physical health team and be instrumental in further development of this service.

Main duties of the job

To provide primary care input for patients working collaboratively with inpatient multidisciplinary teams
Health promotion
Management of chronic diseases
service evaluation and improvement projects, with opportunities to work with colleagues to improve the primary care provision for our inpatient services.
