Job summary
We are looking for an experienced and enthusiastic pharmacist for an exciting Advanced Pharmacist Practitioner to develop and deliver clinical pharmacy services to the Newcastle and Gateshead Universal Crisis Resolution and Home Team (UCRHT), based at St Nicholas Hospital and the Psychiatric Liaison Team (PLT) based at Newcastle Upon Tyne Hospitals NHS FT. You will also support the wider pharmacy team in the provision of clinical pharmacy services within in the Trust.
We provide pharmacy and medicines optimisation services to all our mental health, learning disability and neuro-rehabilitation inpatient areas, as well as to community mental health teams, crisis teams, psychiatric liaison and a variety of other specialist areas. Pharmacy staff are regarded as valued members of the multi-disciplinary team. We have several pharmacist independent prescribers, who are active within a range of clinical services.
Leading and delivering medicines optimisation to patients in the Newcastle and Gateshead UCRHT and the Newcastle PLT.
Providing expert information regarding medicines and medicines optimisation to a range of audiences including service users and carers, Trust staff, acute Trust staff, and primary care.
Main duties of the job
Complete medication and clinical history reviews to rationalise and optimise the safety and effectiveness of pharmacological treatment regimens, considering physical, mental health and general lifestyle needs for patients accepted into the UCRHT Team for home-based treatment and patients referred by the PLT.Participation in multidisciplinary clinical team meetings.
Undertake structured medication reviews on a referral basis for patients with complex pharmaceutical needs.
Provide advice on specific interventions following both national and local guidance with appropriate follow up.
Participating in management/leadership and supervision of staff and contributing to the wider operational services of the pharmacy department.
Support teams with shared care and formulary prescribing issues, liaising with primary and secondary care.
Ensuring services in own area are patient-centred and are delivered in a manner which meets the needs of patients and other stakeholders.
Developing and supporting the development of services in order to meet changing operational or clinical needs.
To personally work and encourage others to work within standards, legislation and guidelines in keeping with professional registration requirements.
To act as a role model for other staff and to demonstrate personal conduct in a manner in keeping with being a registered professional.
