Job summary
An exciting and rare opportunity has arisen within the South Tyneside Learning Disability and Mental Health Services across Lifespan for a Band 7 Specialist Nurse on a substantive basis.
The Specialist Nurse works within South Tyneside Learning Disability and Mental Health Services across Lifespan ranging from children to older persons. The aim of this role is primarily to support reduction in hospital admissions for people with Learning Disabilities, Autism or a Mental Health condition where less restrictive intervention can be facilitated within the community. Where there is a need for hospital admission for active treatment, it is expected that a robust discharge plan is formulated to support the safe and effective transfer back into the community for the individual.
Main duties of the job
- Facilitate collaborative working with individuals and families to enhance quality of care and promote person centred outcomes.
- Work collaboratively with statutory and 3rd Sector providers.
- Facilitate proactive work within the Mental Health and Learning Disability Pathways to increase resilience across community services to reduce the need for hospital admission.
- Take a lead role in maintaining the Dynamic Support Register for individuals who have Autism and/or Learning Disability and complex support needs.
- Chair Care and Treatment Reviews (CTR) or Care Education and Treatment Reviews (CETR) on behalf of the Joint Commissioning Unit.
- Provide oversight and scaffolding in collaboration with the MDT in relation to Complex Cases where the individual is at risk of hospital admission or placement breakdown from the point of referral for CTR in respect of the learning disability pathways and admission in the mental health pathways.
- Undertake Quality Oversight Visits on behalf of the Joint Commissioning Unit for individuals with a learning disability and/or Autism who are in hospital.
- Report to the Joint Commissioning Unit and NHS England in relation to CTR/CETR outcomes and Quality Oversight Visits.
- Support effective communication and collaborative working within Community and Inpatient Learning Disability and Mental Health Services to facilitate smooth and timely discharge from hospital.
