We are pleased to offer an exciting opportunity within the Universal Crisis Team Younger Person's Pathway. The team have recently began to work within Crisis model and as the service that continues to evolve and develop we look to recruit a community practitioner who is passionate about the role and who will be encouraged to be involved in service improvement and the delivery of care ensuring an enhanced, seamless service.
Main duties of the job
The Younger Person Pathway of the Crisis Team offer advice, triage, offer initial assessment and follow up as well as home treatment for under 18s experiencing a mental health crisis, as an alternative to hospital admission. The post holder will have clinical expertise in assessment, intervention and evaluation in mental health care. The role will involve the psycho-social assessment of those who have presented in crisis. They will be required to provide specialist advice and support to service users, carers and other practitioners.
The successful candidates will have responsibility for the effective management of a shared caseload of children and young people referred into the service. The service provides the following functions; urgent triage, assessment for people in crisis, home based treatment, facilitating early discharge from hospital as well as supporting with 136. The service operates 24 hours a day, 7 days a week including bank holidays and weekends via shift working.The applicant should hold a professionally registered qualification and have experience of working within a mental health/ learning disability area.
