A fantastic opportunity has arisen within the Adult Community Learning Disability team for a community practitioner to join our team.
A team made up of a wide range of MDT principles with a friendly specialist approach to supporting individuals with a learning disability maintain independence within the community. Over coming health inequalities and promoting well being our team has the fortunate opportunity to enhance the lives of our patients
Main duties of the job
To manage case work under direction of care co-ordinator/ Pathway ClinicalLead and provide ongoing Assessment and Recovery focused Care Planning for service users who do not have enhanced needs and where risks are assessed as low, under the supervision of the Pathway Clinical Lead.To provide, under the direction of a Care Coordinator and or Pathway Clinical Lead, effective treatment and intervention to service users with complex and substantial needs.To deliver evidenced based clinical and psychologically based Interventions, with appropriate Training and supervision and as directed by Pathway Clinical Lead and Supervisor.The post holder will work as a member of the multi-disciplinary team ensuring that high quality, individualised care is delivered and that this promotes Recovery and Wellbeing and maximises Independence. To provide compassionate care that is based on empathy, kindness, respect and dignity.Ability to work flexibly to meet the needs of Service Users across pathways which may regularly include working flexibly within service hours. Demonstrate flexibility in the role. This may involve working across clinical pathways to support the needs of the service, whilst ensuring the delivery of high quality care at all times.

- First Level Registration RMN/RNMH/RNLD Qualification
- Experience of working with service users and carers in successful delivery of treatment plans
- Proven ability to be able to communicate interventions to service users.
Desirable
- Previous experience in a community setting.
- Experience of involving service users/ carers in evaluation/planning and monitoring of services
Knowledge and Experience
Essential
- Knowledge of relevant contemporary professional theories of mental health care and evidence based interventions.
- Understanding of the statutory issues which impact on service users.
Desirable
- 12 months post registration experience, and completed preceptorship

- Ability to use IT system
- Comprehensive continuous assessment of skills
- Risk Management and Safety Planning skills.
