Job summary
The Northern Region Gender Dysphoria Service (NRGDS) is a national service offering assessment and treatment for people with gender dysphoria. The service offers psychologically informed medical care, voice and communication therapy and psychological therapy as well as onward referral for gender affirming surgeries and hair reduction.
In a period of exciting change and development we are looking to recruit for the post of Community Nurse. This post is suitable for registered adult, learning disability or mental health nurses provided you have the required experience and skills. Due to this being a new post this is a fixed term contract for 12 months, with plans to be reviewed with the hope of advertising a permanent post.
The role of the Community Nurse is offering support to people waiting for surgery and helping them to navigate the care pathway, with regular reviews and support. Additionally the Community Nurse will help ensure that people meet their goals through signposting, liaising with healthcare systems and services.

Main duties of the job
- Working independently to manage a caseload of people waiting for gender affirming surgery and acting as the Named Professional for these individuals
- Following the individual care plan and supporting the service user to do the same in collaboration with the individual's Lead Clinician
- Arranging review appointments in keeping with service specification and the service user's point in their care pathway
- In collaboration with service users, reviewing their mental and physical health, surgical choices, wellbeing and progress towards goals.
- Producing written records of reviews in the electronic patient record and completing all relevant paperwork associated with reviews, including letters to general practitioners surgeons and other relevant parties.
- Signposting to other services for support
- Using clinical supervision effectively and contributing to annual reviews
- Ensuring that health monitoring results are available to the MDT
- Arranging reviews with the Lead Clinician.
