The North Cumbria Crisis Resolution and Home Treatment Team are making some exciting changes and welcome applications for Band 6 Community Practitioners. There are vacancies in both the East (Carlisle) and West (Lillyhall) Teams.
There are opportunities for full time and part time employment, working alongside a dedicated and enthusiastic multidisciplinary team.
The post holder will be part of a team working under the crisis model and pathway, offering a 24/7 service as an alternative to hospital admission. The successful candidate must have experience of working with adults and ideally have a track record of clinical experience in working with risk and complexity.
The role requires sound clinical skills in all aspects of triage, assessment, formulation, management of risk and therapeutic interventions.
This opportunity also comes with the possibility of up to a £8000 relocation package available. Which can be discussed at interview.
Main duties of the job
Practitioners will be expectedto triage referrals which will involve arranging face to face assessments, safety planning and sign posting to the appropriate service.
Practitioners will spend time as part of the assessment team. This will involve completing comprehensive mental health and social needs assessments in patients' homes. Gatekeeping assessments for hospital admission. Care planning for Home Treatment interventions. Risk assessments and safety planning.
Practitioners will be involved in Home Treatment interventions and reviews, working with patients and their support network to reduce the need for hospital admission and any deterioration in mental health. Safeguarding and managing risk is a large role of a Practitioner.
North Cumbria CRHT work as part of an MDT, Practitioners will be expected to play an active role in making decisions and raising concerns.

At all times work in collaboration with Service Users, and Carers ensuring that the Service User is treated with Compassion, Respect and Dignity. Assess, Manage and Evaluate Risk as an ongoing process using the Risk Assessment Tool identified by the Trust.
Escalate Risks and Communicate effectively with Partners and seek advice from more senior staff and Teams such as Safeguarding, MAPPA, MARAC as appropriate.
Manage distress and conflict in the day to day work with Service Users who, as an expression of their distress, may at times be difficult to engage and/or demonstrate significant risk behaviours.
Provide effective Care and Therapeutic Interventions to Service Users presenting with a range of Difficulties. To accept the role of Lead Professional as appropriate to the needs of an individual and demonstrate acomprehensive knowledge, understanding and application of the role in accordance with Trust Care Coordination Policy.To engage in active relapse prevention planning with service users and carers, contributing to their recovery, wellbeing and discharge planning.Coordinate any transitions across Pathways provided by the Trust, ensuring high level of communication is maintained.
