Primary Mental Health Care Practitioner - Valens PCN

Main duties of the job

An exciting opportunity has arisen for an experienced mental health clinician to work as a Primary Care Mental Health Practitioner within Valens Primary Care Network (PCN) a leading, innovative GP practice with over 50,000 patients based in Northumberland. You will become part of a team; focusing on supporting the needs of our local population living with mental health and wellbeing issues.

Valens is leading on the development of an integrated care model for adult mental health and has established a new and innovative service by working in a collaborative and multiagency way with our local partners to meet the needs of patients with a range of mental health needs.

In this role you will be working within a multi-disciplinary team; alongside other PCN roles including mental health nurse practitioners and occupational therapists, clinical pharmacists and social prescribers, as well as community mental health, social care, pharmacy, hospital and voluntary sector colleagues; focusing on a personalised care approach to achieve the best possible care outcome for patients. You will become part of our experienced healthcare team, supporting our core GP services by offering assessment, brief interventions and navigation to local services to both working age adults and older patients. You should have extensive clinical knowledge and experience across a range of mental health settings.

You will be employed under Agenda for Change terms and conditions and be based at the Valens practice whilst receiving supervision and line management via Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Trust. This arrangement affords full access a range of CNTW employee benefits as well as excellent support and development opportunities available through both the PCN and CNTW. Successful candidates will have the opportunity to work flexibly within GP contracted hours between 8am and 8pm Monday - Friday and 9am - 1pm on Saturdays. You will be expected to contribute where possible to the enhanced opening access by providing a late clinic per week and occasional Saturday clinics.

Essential

  • First Level Nursing Qualification and Registration or equivalent
  • Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post
  • Proven track record of post registration clinical experience relevant to the post.
  • Up-to-date knowledge/experience of relevant legislation.

Desirable

  • Post Registration Qualifications in Psychological Interventions
  • Experience of working within a community team.
