East Memory and Later Life Services (MLLS) are excited to welcome an enthusiastic, compassionate and dedicated community mental health nurse (Band 5) to our team, to support in making a significant impact on the well-being of older adults in Carlisle and Eden.
Main duties of the job
Our service are committed to providing outstanding care and support to older adults in our community who are experiencing memory issues, co-morbid functional mental illness and complex/age related needs. We believe in the power of person - centred care, and our team are driven to create a welcoming and inclusive environment where every individual feels valued and respected.
Our practitioners work across three pathways: Early Memory and Wellbeing Pathway, Complex Needs Pathway, and CHESS pathway. This post will sit within the Early Memory or Complex pathway. Your expertise and compassion will enable you to build strong therapeutic relationships with patients and carers/families, to help empower individuals to enhance their quality of life and explore available support.
You will work collaboratively with a multidisciplinary team of professionals, to assess the needs of our patients, support in assessment towards diagnosis of dementia, and deliver evidence based treatment and interventions as addressed in individuals care plans.

- First Level Registration RMN/RNMH/RNLD Qualification Experience of working with service users and carers in successful delivery of treatment plans Multi-disciplinary team work. Proven ability to develop therapeutic partnership with service users and carers. Proven ability to be able to communicate
Desirable
- Further relevant post qualifying training Previous experience in a community setting. Experience of involving service users/ carers in evaluation/planning and monitoring of services
Knowledge and Experience
Essential
- 12 months post registration experience, and completed preceptorship Knowledge of relevant contemporary professional theories of mental health care and evidence based interventions. Understanding of the statutory issues which impact on service users.
Desirable
- Knowledge and experience working with older adults with memory/complex function mental health needs

- Ability to use IT system Comprehensive continuous assessment of skills Risk Assessment skills. Risk Management and Safety Planning skills.
