Job summary

Memory Protection Service

An excellent opportunity has arisen to join the Memory Protection Service South of Tyne. The Memory Protection Services is an early diagnostic service for those individuals who may be suffering from a Dementia. It covers a wide geographical area including Gateshead, South Tyneside and Sunderland.

Assessments take place in local Health Centres in all three areas and in service users homes.

The Memory Protection Service is an ageless service. It laises closely with stautorty and non stautory agencies to ensure the best care for the service user.

Working in a multi disciplinary team, the role includes triage of referrals, completing a comprehensive assessment for the service user, supporting with titration, supporting service users post diagnostically and offering cognitive therapeutic approaches.

Main duties of the job

To be responsible for the Assessment of individuals who may be suffering from a Dementia.Provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, 3rd sector partner agencies and the public.To help and support the Service User and Carer during the journey from assessment to diagnosis and treatent where appropriate.To consider Carer needs and ensure Carer and family are involved in the whole process from assessment to diagnosis and treatment.To deliver evidenced based Clinical and Psychologically based Interventions.The post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment Living Well with Dementia,promoting wellbeing and maximising independence.Support with the Titration of prescribed medication.To provide person centred, compassionate care that is based on empathy, kindness, respect and dignity.To incorporate a person centred philosophy into nursing practice and client centred and collaberative care.

Knowledge and Experience

Essential

  • 12 to 24 month band 5 qualified nurse experience dependent upon service need.
  • Awareness and understanding of professional accountability, NMC standards and other local and national initiatives.

Skills and Competencies

Essential

  • Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs.
  • Risk Assessment and Risk Management / Safety Planning skills.
  • Demonstrate an understanding of effective & appropriate treatment options including a range of evidence based clinical approaches as identified in N.I.C.E. Guidance).
  • Demonstrate effective written and verbal communication skills.
  • Evidence of a high level of professional, organisational and leadership skills (self & others).
  • Ability to adapt within a changing environment.
  • Ability to work under pressure.

Education and Qualifications

Essential

  • First Level Registered Nurse RNMH, RMN, RNLD or equivalent first level qualification e.g. Social Worker, OT, Clinical Psychologist, other.
  • Mentor Training.
  • Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
  • Proven track record of post registration clinical experience relevant to the post.
  • Up-to-date knowledge/experience of relevant legislation.

  • Previous Community experience
  • Experience in working with Older Adults
  • Understand the range and complexity of a diagnosis of Dementia
  • Teaching/Presentation skills
  • Delegation skills.
