Job summary

This is an exciting and unique opportunity for an experienced and highly motivated person to be a part of both CNTW Children & Young Peoples (CYPS) Sunderland team as well as working in Sunderland's Autism Outreach Team (AOT), which is now part of Together for Children's SEND Specialist Support Service . This 12 month fixed term post has been created to bring health and education together in order to support the emotional and mental health needs of children and young people with Autism across the city.

Main duties of the job

The successful candidate will be based predominantly with the Autism Outreach Team and will complete direct work with children and young people in education settings as well as delivering relevant training to key stakeholders. In addition, they will play an integral role in the neurodevelopmental pathway formulation meetings as a specialist nurse and receive regular clinical supervision and CPD opportunities from CYPS.

Job responsibilities

Key Responsibilities:

  • To advise and support the work of the SEND Specialist Support Services Team at Together for Children.
  • To work with schools, parents, key practitioners and children / young people (CYP) with Autism in promoting and supporting the effective inclusion of CYP attending mainstream schools.
  • To contribute towards the assessment of the individual needs of CYP with a diagnosis of Autism and SEMH needs.
  • To provide support for CYP, teachers and the whole school as outlined below:

* Raising awareness to all staff of the strengths and difficulties of individual pupils with Autism.

* Assisting all staff to identify the needs of CYP with Autism to develop effect approaches to address their mental health and emotional wellbeing need

* Preparing and reviewing student targets as created by Autism Outreach Team (AOT).

* Modelling resources and equipment required to address students needs and support their participation in learning tasks and activities.

* Assisting with the planning and programming of specific transition activities.

* Using specialist knowledge to work with individual students and groups to support their understanding of mental health, physical health and emotional wellbeing and to support parents/ carers, school staff and other relevant stakeholders through delivering training packages and interventions

  • The successful post holder will also work into CNTW Children & Young Peoples (CYPS) Sunderland team for part of this role providing formulation and clinical support as required, the post holder will receive regular clinical supervision from within CYPS.
