Main duties of the job

The post holder will act as a Named Nurse within a multi-disciplinary team, developing coproduced, patient centered needs based care plans, through assessment, planning, implementation and evaluation.

You will provide excellent professional, skilled and effective person centered, evidence based nursing care.

You will be actively involved in the health and wellbeing of staff, service users, families and carer's.

You will be visible and accessible to patients, families and carers working together to ensure efficient, effective safe and caring services.

You will contribute to achieving all performance indicators, essential standards and quality outcomes.

You will contribute to the education and development of the workforce and students.

You will participate in staff appraisals and support personal development plans.

You will safely administer medication following Trust policies and practice guidelines.

You will provide compassionate care that is based on empathy, kindness, respect and dignitythe post holder will work as part of the multi-disciplinary team and will be part of the assessment of care needs, risk assessment, formulation & implementation of care plans that are meaningful to our Patients.
