Job summary
An exciting opportunity has arisen within the Northumberland Older Persons Psychiatric liaison Service for the role of Community Practitioner.
We are looking to recruit a caring and compassionate community clinician with excellent interpersonal and communication skills to work as part of our multi-disciplinary team offering assessment and treatment to clients who are requiring liaison input whilst receiving medical treatment in the local acute hospital.
Your responsibilities will include undertaking a biopsychological assessment and risk assessment, carrying out reviews and liaison with other professionals, family members and out the trust such as local authority.
The role requires an element of autonomous working and leadership skills, and you must be able to demonstrate a strong commitment to the delivery of high quality, safe and effective care and treatment.
It is essential that you have the relevant professional qualification .
Main duties of the job
To be responsive to referrals and provide a biopsychosocial assessment including risk and identifying ongoing needs.
Provide specialist clinical advice to Service Users/Carers, acute care staff, statutory agencies such as local authority .
To identify ongoing liaison interventions and provide evidenced based clinical and psychological interventions.
To consider Carer needs and ensure Carer and family are involved in Care Planning where appropriate.
Contribute to the multidisciplinary meetings.
Contribute to education of acute care staff.
