An exciting opportunity has arisen within the Sunderland Psychological Wellbeing Service to appoint a Band 6 Community Practitioner on a substantive basis.
The service is currently delivered within an integrated model of Primary Care Mental Health and Improving Access to Psychological Therapy (IAPT) which is provided in partnership between Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust, Sunderland Counselling Service and Washington Mind.
We are a dynamic and dedicated team that offers a range of psychological therapies and services to treat people and support their recovery within a stepped care model. The team is made up of around 80+ clinicians including Psychological Wellbeing Practitioners (PWP's), High Intensity Therapists, Community Psychiatric Nurses, a Dual Diagnosis Specialist, Employment Support Advisors and a Counsellor. We are an innovative service and deliver treatment via telephone and online consultation, alongside face to face appointments, so you must be confident in the use of IT.
Main duties of the job
As a Band 6 Community Practitioner you will work within the Primary Care Mental Health element of the Team and will provide evidenced based interventions, as identified within NICE guidelines, to a caseload of patients from across our locality who are experiencing mild to moderate common mental health problems. You will act as the Referral Co-ordinator and screen any new referrals coming into the service for appropriateness alongside offering advice to anyone from outside of the Team with regards to what we can offer as a service.

Education and Qualification
Essential
- First Level Registered Nurse RNMH, RMN, RNLD or equivalent first level qualification e.g. Social Worker, OT, Clinical Psychologist, other.
- Mentor Training
- Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
- Proven track record of post registration clinical experience relevant to the post.
- Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection
Desirable
- Post Registration Qualifications in Psychological Interventions Experience of working within a community team. Non-Medical Prescribing
- Experience of working within a community team.
Knowledge and Experience

- Minimum two years' experience post registration.
