A rare and exciting opportunity has arisen for a permanent, full time specialist Band 7 Clinical Lead to work in the Adult Learning Disabilities Team in North Cumbria.
You will join a well established yet expanding team with a full MDT complement. The team operates within a pathway approach and we are looking for an experienced nurse to lead the physical health pathway.
Main duties of the job
You will work to reduce inequalities in healthcare and maximise health outcomes for adults with varying degrees of learning disability and additional complex need.
You will work directly with individuals who have complex needs, their families, carers, providers, statutory and non statutory organisations. You will assess, plan and implement appropriate individualised care. You will lead complex case reviews and MDT's, balance risk and contribute to the safe and effective delivery of care.
We are looking for a registered nurse with learning disability experience.
About us
The team is located across North Cumbria and has rapidly expanded over the last eighteen months. The team works within a pathway approach consisting of mental health, forensic, behaviour and physical health. Also within the team we have psychiatry, psychology, OT, SLT and physiotherapy. We have a range of nurses, assistant practitioners, carer leads and administrative staff that provide support across the service. The team is supportive of each other, and, passionate regarding people with learning disabilities to live their lives within their own maximum ability.
