Motor Neurone Disease Care Coordinator – North and West

Job summary

The Motor Neurone Disease (MND) Care Coordinator will work with adults diagnosed with MND, a progressive neurological disease. These people have complex, predictable and unpredictable palliative, emotional, psychological and physical care needs.

The post holder will be a highly skilled professional in this specialist area of practice who concentrates on ensuring health, social care and third sector services for people affected by MND are optimised. This will be achieved by:

Coordinating comprehensive care between many different teams and organisations;

Working autonomously and creatively across boundaries to implement change and improve patient care;

Establishing clear care pathways for people at all stages of their disease;

Promoting and providing individualistic holistic care to all people affected with MND.

The role will work with residents in North and West Cumbria diagnosed with MND, many of who are under the provision of the Regional MND Care and Research Centre at the RVI in Newcastle. Some people in the area are linked to other MND Care Centres due to geography and choice. The post is funded by Newcastle Hospitals and hosted by North Cumbria Integrated Care Trust to provide an effective and timely service for people with MND in North and West Cumbria.

Main duties of the job

  • Develop specialised care for a defined caseload supporting people with MND along clinical pathways and protocols of care with reference to current NICE approved management guidelines.

Encourage people with MND to be partners in their own healthcare through education and discussion to facilitate choice and involvement in decision making.

Plan and coordinate the input of services to ensure the complex needs of the person with MND are fully met with timely and appropriate referrals.

Work as an integral member of the MDT to advise and support colleagues in the delivery of specialist patient care which is cost effective and optimises outcomes for people with MND and their families.

Undertake or advise on the management of a specific aspects of care associated with symptom management such as mobility issues, breathing, sections and dysphagia; as well as early access/liaison with palliative care.

Assess, analyse and action interventions as needed to meet patients' priorities of care.

Use a significant level of autonomy to regularly review and competently evaluate peoples' needs enabling revision of care and treatment options.

Facilitate and lead bi-monthly MDT's to monitor disease progression, provide evidence based advice and coordinate care while acting as a patient advocate.
