Job summary
The Children in Care and Youth Justice service are jointly commissioned by North East and North Cumbria Integrated Care Board and Lancashire and South Cumbria Integrated Care Board. The service operates a countywide model and is responsible for:
- Supporting the health needs of children and young people open to the Youth Justice Service and Criminal Justice Liaison and Diversion (CJL&D) service supporting vulnerable young people whilst in contact with the criminal justice system.
The post holder will be primarily responsible for supporting the health needs of children and young people open to the youth justice service. They will also be required to work with children and young people who are subject to a care order, promoting seamless healthcare.
This post covers North Cumbria, the base will be confirmed with the successful candidate.
There will be an expectation that the post holder will also work from Youth Justice bases in Workington and Carlisle.
Main duties of the job
The post holder will be expected to:
Manage an individual clinical caseload
Be able to assess holistic health and wellbeing needs using a trauma informed approach
Develop and implement brief care package for common health needs- for example sleeping, lifestyle, sexual health, life skills, low mood/anxiety/emotional regulation/stress management and evaluate their effectiveness.
Coordinate health services across a range of providers, promoting seamless, continuous health care
Help develop specialist pathways of health support to improve health outcomes for children within the criminal justice system.
Act as the health primary point of contact for young people, their families and professionals
Foster effective working relationships with partner agencies in order to promote interagency collaboration
Support the Named Nurse and Clinical Lead Manger in meeting their statutory and commissioned responsibilities.
Support continuous service development.
Work autonomously as well as part of team
